TY - JOUR AU - Wongkpratoom, Suporn AU - Srisuphan, Wichit AU - Senaratana, Wilawan AU - Nantachaipan, Pikul AU - Sritanyarat, Wanapa PY - 2013/02/06 Y2 - 2024/03/29 TI - Role Development of Advanced Practice Nurses in Thailand JF - Pacific Rim International Journal of Nursing Research JA - PRIJNR VL - 14 IS - 2 SE - Original paper DO - UR - https://he02.tci-thaijo.org/index.php/PRIJNR/article/view/6302 SP - 162-177 AB - <p><strong>บทคัดย่อ</strong></p><p>ผู้ปฏิบัติการพยาบาลขั้นสูงเป็นเรื่องใหม่ของการได้รับรองวุฒิบัตรสำหรับพยาบาลในประเทศไทยโดยสภาการพยาบาลเมื่อ พ.ศ. 2546 ตั้งแต่มีบทบาทผู้ปฏิบัติการพยาบาลขั้นสูงในประเทศไทยยังไม่พบการศึกษาวิจัยเกี่ยวกับการพัฒนาบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูง ดังนั้นวัตถุประสงค์ของการวิจัยครั้งนี้ เพื่อศึกษาการพัฒนาบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูงในประเทศไทย โดยใช้ระเบียบวิธีวิจัยแบบผสมผสาน 2 ระยะ ระยะแรกใช้วิธีการศึกษาเชิงปริมาณโดยใช้แบบสอบถามเพื่อสำรวจผู้ปฏิบัติการพยาบาลขั้นสูงที่ได้รับวุฒิบัตรจากสภาการพยาบาล ตั้งแต่ พ.ศ. 2546 – 2548 จำนวน154 คน ระยะที่สองใช้วิธีการศึกษาเชิงคุณภาพเพื่อศึกษาข้อมูลจากผู้ให้ข้อมูลจำนวน 13 คน โดยใช้วิธีการสัมภาษณ์เชิงลึก การสังเกตอย่างไม่มีส่วนร่วม การบันทึกภาคสนาม และการทบทวนเอกสารวิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและวิเคราะห์เชิงเนื้อหา</p><p>ผลการศึกษาระยะแรกพบว่า การปฏิบัติบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูงมีมาก คือ บทบาทของผู้ปฏิบัติการพยาบาล ผู้ให้ความรู้ ผู้ให้คำปรึกษา ผู้จัดการ และผู้วิจัย ในขณะที่การปฏิบัติบทบาทด้านจริยธรรมและกฎหมายมีปานกลาง ผลการศึกษาระยะที่สองพบว่า กระบวนการพัฒนาบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูงประกอบด้วย 3 ขั้น ได้แก่ ผู้เริ่มต้น ผู้มีความสามารถ และผู้เชี่ยวชาญ ปัจจัยสนับสนุนที่สำคัญของการพัฒนาบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูง คือ ก) ปัจจัยด้านองค์กร (ระบบบริการสุขภาพและนโยบายองค์กร) ข) ปัจจัยด้านบุคคล (คุณลักษณะของผู้บริหารการพยาบาล และการปฏิบัติงานที่ดีของทีมสหสาขาวิชาชีพ) และ ค) ปัจจัยแหล่งสนับสนุน (การได้รับทุน) ปัจจัยอุปสรรคที่สำคัญที่สุดในการพัฒนาบทบาทของผู้ปฏิบัติการพยาบาลขั้นสูง คือ ก) ปัจจัยด้านองค์กรเกี่ยวกับการบริหารงานที่ไม่ดี(ขาดคำอธิบายโครงสร้างองค์กรที่ชัดเจน และนโยบายขององค์กรไม่ชัดเจน) ข) ปัจจัยด้านบุคคลเกี่ยวกับการสนับสนุนจากผู้บริหารด้านการปฏิบัติการพยาบาลขั้นสูงน้อย (การมอบหมายงานที่ไม่สะท้อนการปฏิบัติการพยาบาลขั้นสูง และพฤติกรรมไม่ให้ความร่วมมือของสมาชิกในทีมสหสาขาวิชาชีพ และ ค) ปัจจัยแหล่งสนับสนุนเกี่ยวกับการขาดอัตรากำลังพยาบาล (การมอบหมายงานที่ไม่ใช่การปฏิบัติการพยาบาลขั้นสูง)</p><p><strong>คำสำคัญ:</strong> การวิจัยแบบผสมผสาน; การปฏิบัติและการพัฒนาบทบาท; ผู้ปฏิบัติการพยาบาลขั้นสูง;ประเทศไทย</p><p> </p><p><strong>Abstract</strong></p><p>Advanced practice nurse (APN), a relatively new form of certification for nursesin Thailand, was approved, in 2003, by the Thailand Nursing and Midwifery Council (TNMC).Since inception of the APN role, in Thailand, study of its development has not been undertaken.The purpose of this study, therefore, was to explore, through use of a two-phase sequentialmixed method design, role development of APNs in Thailand. The first phase employeda quantitative method, using self-reported questionnaires, to survey 154 APNs who hadbeen certified, between 2003 and 2005, by the TNMC. The second phase utilized aqualitative method to seek information from 13 participants through use of in-depthinterviews, non-participant observation, field notes and document review. Data wereanalyzed via descriptive statistics and content analysis.</p><p>Findings from the first phase revealed APN performance was high within the rolesof direct clinical care, educator, consultant, administrator and researcher, while performancewithin the role of ethicist/legalist was moderate. Results from the second phase revealedAPN role development was comprised of three stages: advanced beginner, competent practitionerand expert. The major facilitating factors of APN role development were found to be: a)organizational (healthcare system and organizational policies); b) human (quality nurseadministrators and well-functioning multidisciplinary teams) and c) resources (financialassistance). The greatest barriers in role development were the: a) organizational factor ofpoor administrative functioning (lack of a clearly delineated organizational structure andunclear organizational policies); b) human factor of poor administrative support for advancedpractice nursing (work assignments not reflective of advanced practice nursing and uncooperativebehavior by members of multidisciplinary teams); and, c) resource factor of a nursingshortage (work assignments in non-advanced practice situations).</p><p><strong>Keywords: </strong>Mixed methods research; Role performance and development; Advancedpractice nurses; Thailand</p> ER -
