TY - JOUR AU - Shangyom, Duangtida AU - Detprapon, Mukda AU - Malathum, Porntip PY - 2019/01/07 Y2 - 2024/03/29 TI - Self-Care Behaviors in Older Persons with Upper Respiratory Tract Infection JF - Nursing Research and Innovation Journal JA - Nurs Res Inno J VL - 24 IS - 3 SE - บทความวิจัย DO - UR - https://he02.tci-thaijo.org/index.php/RNJ/article/view/143432 SP - 345-360 AB - <p><strong>บทคัดย่อ</strong><strong>:</strong></p><p>การวิจัยเชิงบรรยายครั้งนี้มีวัตถุประสงค์เพื่อศึกษาพฤติกรรมการดูแลตนเองของผู้สูงอายุที่ติดเชื้อทางเดินหายใจส่วนบนทั้งด้านการไม่ใช้ยาและด้านการใช้ยา โดยใช้ทฤษฏีการดูแลตนเองของโอเร็มเป็นกรอบแนวคิดในการวิจัย กลุ่มตัวอย่างเป็นผู้สูงอายุที่มีอายุตั้งแต่ 60 ปีขึ้นไปที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นโรคติดเชื้อระบบทางเดินหายใจส่วนบนที่เข้ารับบริการที่แผนกผู้ป่วยนอก โรงพยาบาลทุติยภูมิแห่งหนึ่งในกรุงเทพมหานคร คัดเลือกแบบเฉพาะเจาะจง จำนวน 105 ราย ระหว่างเดือนตุลาคมถึงเดือนพฤศจิกายน พ.ศ. 2560 เก็บข้อมูลโดยใช้แบบสัมภาษณ์กึ่งโครงสร้าง วิเคราะห์ข้อมูลโดยใช้สถิติบรรยาย ผลการวิจัยพบว่า กลุ่มตัวอย่างมีพฤติกรรมการดูแลตนเองทั้งด้านการไม่ใช้ยาและด้านการใช้ยาด้วยรูปแบบและวิธีที่หลากหลายผสมผสานกัน รวมทั้งมีการแสวงหาความช่วยเหลือจากเจ้าหน้าที่สุขภาพและแหล่งบริการสุขภาพ ตลอดจนมีการป้องกันการแพร่กระจายเชื้อโรคไปสู่บุคคลอื่น กลุ่มตัวอย่างเกือบทั้งหมด (ร้อยละ 90) มีพฤติกรรมการใช้ยาหลายชนิดร่วมกัน ส่วนใหญ่มีการใช้ยาด้วยตนเองก่อนมาพบแพทย์ ซึ่งประมาณครึ่งหนึ่งเป็นยาที่ไม่มีใบสั่งยาจากแพทย์ ส่วนใหญ่แสวงหาความช่วยเหลือเกี่ยวกับการใช้ยาจากหลายแหล่ง ในขณะที่บางส่วน (ร้อยละ 10) ไม่สามารถประเมินความรุนแรงของอาการที่ต้องรีบมาพบแพทย์ พฤติกรรมส่วนน้อย ได้แก่ ซื้อยาใช้เองโดยไม่มีเภสัชกรแนะนำ เพิ่ม ลดหรือหยุดยาปฏิชีวนะหรือยาต้านไวรัสเอง ส่วนพฤติกรรมด้านการป้องกันพบว่า กลุ่มตัวอย่างประมาณ 2/3 ไม่ได้รับการฉีดวัคซีนป้องกันการติดเชื้อทางเดินหายใจเป็นประจำทุกปี ส่วนที่เหลือประมาณ 1/3 ได้รับการฉีดวัคซีนป้องกันไข้หวัดใหญ่ และมีบางรายที่ได้รับวัคซีนป้องกันปอดอักเสบด้วย ผลการศึกษาครั้งนี้ชี้ให้เห็นว่าพฤติกรรมการดูแลตนเองด้านการใช้ยาบางประการของผู้สูงอายุที่ติดเชื้อทางเดินหายใจส่วนบนมีความเสี่ยงต่อการเกิดภาวะแทรกซ้อนของโรค ปัญหาเชื้อดื้อยา การใช้ยาซ้ำซ้อน ปฏิกิริยาระหว่างยา และอาการไม่พึงประสงค์จากการใช้ยา ดังนั้น ข้อมูลจากผลการวิจัยนี้ควรนำไปใช้ในการพัฒนาแนวทางการส่งเสริมพฤติกรรมการดูแลตนเองเพื่อการป้องกันและการจัดการอาการของผู้สูงอายุเมื่อมีการติดเชื้อทางเดินหายใจส่วนบนที่เหมาะสม</p><p>&nbsp;</p><p><strong>คำสำคัญ</strong>: พฤติกรรมการดูแลตนเอง ผู้สูงอายุ โรคติดเชื้อทางเดินหายใจส่วนบน พฤติกรรมการไม่ใช้ยา</p><p>&nbsp;พฤติกรรมการใช้ยา</p><p>&nbsp;</p><p>Self-Care Behaviors in Older Persons with Upper Respiratory Tract Infection</p><p><strong>Abstract:</strong></p><p>The purpose of this descriptive study was to explore self-care behaviors in older persons with &nbsp;upper respiratory tract infection. Orem's self-care theory was used to guide the study. The sample &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;consisted of 105 persons with at least 60 years old who had been diagnosed by a physician as upper &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;respiratory tract infection and were recruited by purposive sampling from an outpatient department of a secondary hospital in Bangkok. The data were collected from October to November, 2017. Semi-structured interviews were used to collect the data, which were analyzed using descriptive statistics. The results revealed that the sample performed both non-pharmacological and pharmacological self-care &nbsp;&nbsp;&nbsp;&nbsp;behaviors, which were various and combined with several methods. They also sought help from health care services and health care providers, as well as performed behaviors to prevent the spread of infection to others. Almost all of the sample (90%) had polypharmacy behaviors; most of them had self-medication before seeing the doctor by which a half of the medications had no prescriptions; two-thirds sought help about medications from multiple sources, while 10% of the sample were unable to assess their severity of the symptoms or to make a decision whether they should see a doctor. On the other hand, the minority bought medications without pharmacist guides, and adjusted or stopped antibiotics by themselves. For preventive behaviors, approximately, two-thirds of the sample did not receive vaccination yearly, while one-third received influenza vaccine, and a few received pneumococcal vaccine. The results of this study point out that some self-medication behaviors of older persons may cause a high risk of complications of the disease, drug resistance, duplicate medication, drug interaction, and adverse drug reactions.
