TY - JOUR AU - แสงงาม, จินตนา AU - ธรรมพนิชวัฒน์, วัลยา AU - เลาหเพ็ญแสง, มงคล AU - พยัคฆเรือง, สุดาภรณ์ PY - 2015/06/08 Y2 - 2024/03/29 TI - ผลของโปรแกรมการสอนเพื่อบรรเทาอาการท้องผูกของผู้ป่วยเด็กหลังผ่าตัดแก้ไขภาวะผิดรูปของทวารหนักและลำไส้ตรง หรือโรคลำไส้ใหญ่ โป่งพองแต่กำเนิด JF - The Journal of Thailand Nursing and Midwifery Council JA - J Thai Nurse midwife Counc VL - 30 IS - 1 SE - Research Articles DO - UR - https://he02.tci-thaijo.org/index.php/TJONC/article/view/34682 SP - 86-98 AB - <p>บทคัดย่อ : วัตถุประสงค์ของการวิจัย: เพื่อศึกษาผลของโปรแกรมการสอนเพื่อบรรเทาอาการท้องผูกของผู้ป่วยเด็กหลังผ่าตัดแก้ไขภาวะผิดรูปของทวารหนักและลำไส้ตรง หรือโรคลำไส้ใหญ่โป่งพองแต่กำเนิด<br />การออกแบบวิจัย: การวิจัยกึ่งทดลอง แบบสองกลุ่มวัดก่อนและหลังการทดลอง<br />วิธีดำเนินการวิจัย: กลุ่มตัวอย่างคือ ผู้ดูแลและผู้ป่วยเด็กที่มีภาวะผิดรูปของทวารหนักและลำไส้ตรง หรือโรคลำไส้ใหญ่โป่งพองแต่กำเนิด มารับการติดตามผลการรักษาที่ Anorectal clinic โรงพยาบาลศิริราช จำนวน 36 คู่ แบ่งเป็นกลุ่มละ 18 คู่ กลุ่มทดลองได้รับโปรแกรมการสอนผ่านสื่อคอมพิวเตอร์ ให้คู่มือกลับไปทบทวน และให้ผู้ดูแลเสริมแรงจูงใจให้ผู้ป่วยเด็กขับถ่ายอุจจาระ โทรศัพท์ซักถามปัญหาและให้คำแนะนำแก่ผู้ดูแล ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ ประเมินความรู้ของผู้ดูแลโดยใช้แบบสอบถามความรู้ ประเมินจำนวนครั้งของการถ่ายอุจจาระและอุจจาระเล็ดติดก้นของผู้ป่วยเด็กด้วยตารางบันทึกการขับถ่าย วิเคราะห์ข้อมูลด้วยสถิติ ANCOVA, Chi-square และ Independent t-test<br />ผลการวิจัย: ผู้ดูแลกลุ่มทดลองมีค่าเฉลี่ยคะแนนความรู้เมื่อ 4 สัปดาห์หลังได้รับโปรแกรมการสอนมากกว่ากลุ่มควบคุม (p &lt; .05) ผู้ป่วยเด็กที่มีอาการท้องผูกในกลุ่มทดลองมีจำนวนไม่แตกต่างกับกลุ่มควบคุม (p &gt;.05) และกลุ่มทดลองมีจำนวนครั้งของอุจจาระเล็ดติดก้นไม่แตกต่างกับกลุ่มควบคุม (p &gt; .05)<br />ข้อเสนอแนะ: ควรนำโปรแกรมการสอนนี้ไปใช้เพื่อส่งเสริมให้ผู้ดูแลมีความรู้ในการบรรเทาอาการท้องผูกของผู้ป่วยเด็กหลังผ่าตัดแก้ไข ซึ่งควรพัฒนาโปรแกรมโดยแยกเนื้อหาการสอนเฉพาะโรค และสอนให้ผู้ป่วยเด็กหลังผ่าตัดฝึกกล้ามเนื้อที่ช่วยในการขับถ่ายด้วย</p><p>Abstract: Objective: To examine the effect of an education program to relieve constipation in pediatric patients after surgical correction of anorectal malformation or Hirschsprung’s disease.<br />Design: A quasi-experimental study with two groups pretest-posttest design.<br />Implementation: The sample was composed of 36 pairs of caregivers and pediatric patients of anorectal malformation or Hirschsprung’s disease who came for follow-up at the anorectal clinic of Siriraj hospital. Subjects were divided into an experimental group of 18 subjects who received an education program via computer presentation, and advice for review at home in which the caregivers were asked to encourage the patients in toilet training. The experimental group also received follow-up phone calls to inquire about problems and offer suggestions to the caregivers. The control group of 18 subjects received only routine care. The knowledge of the caregivers was assessed by a questionnaire. The number of bowel movements and soilings were assessed by a record form for bowel movements. The data were analyzed by ANCOVA, chi-square and independent t-test.<br />Results: The caregivers in the experimental group had a higher mean score of knowledge at 4 weeks after the program than those in the control group (p &lt; .05). The number of patients in the experimental group who had constipation was not different from that of the control group (p &gt; .05) and the number of soilings in the experimental group was not different from that of the control group (p &gt; .05).<br />Recommendations: The education program should be used to help enhance caregivers’ knowledge of how to relieve constipation in patients after surgical correction and an education program with specific disease content should also be developed. The patients should be taught to practice the muscles that help with defecation after surgery.</p> ER -
