Physical examination, pathological report, and pelvic node status were evaluated for correlation.<br /><strong>Results:</strong> 19 patients recruited for study; 14 were high risk and 5 were low risk. Mean age of patients was 53 years old. The data separately collected of each side of the lymphatic tract. Palpable noded size ranged from 0-6 cm. Pathological report of positive pelvic node of 5 lymphatic tract. At cut off point smaller than 2cm, there was 1 positive pelvic node (3.4%) and negative predictive value of 97%. Number of inguinal node metastasis ranged from 0-9 node. At cut off point of fewer than 2 metastatic inguinal node, there was 1 positive pelvic node (3.7%) and negative predictive value of 96%<br /><strong>Conclusion:</strong> The patients diagnosed of squamous cell carcinoma of penis with palpable inguinal node smaller than 2 cm had lower chance of pelvic node metastasis. However, this is a pilot study, there were not enough patients recruited to demonstrate the differences between the groups. More participants are required to prove the results valid.<br /><br /><strong>ความสัมพันธ์ของขนาดและจำนวนต่อมน้ำเหลืองบริเวณขาหนีบกับอุบัติการณ์การแพร่กระจายของมะเร็งอวัยวะเพศชายมาต่อมน้ำเหลืองอุ้งเชิงกราน</strong><br />พฤกษ์ ธำรงรัตนศิลป์, ณัฐพงศ์ วงศ์วัฒนาเสถียร<br />หน่วยศัลยศาสตร์ยูโร กลุ่มงานศัลยศาสตร์ โรงพยาบาลราชวิถี<br /><strong>บทคัดย่อ</strong><br /><strong>วัตถุประสงค์:</strong> เพื่อศึกษาอุบัติการณ์การแพร่กระจายมะเร็งอวัยวะเพศชายมาที่ต่อมน้ำเหลืองที่เชิงกรานในผู้ป่วยที่ตรวจร่างกายได้ต่อมน้ำเหลืองบริเวณขาหนีบขนาดต่าง ๆ กัน<br /><strong>ผู้ป่วยและวิธีการศึกษา:</strong> รวบรวมข้อมูลจากแฟ้มประวัติผู้ป่วยที่วินิจฉัยมะเร็งอวัยวะเพศชายและได้รับการผ่าตัดเลาะต่อมน้ำเหลืองตั้งแต่ปี พ.ศ. 2551-2556 ผลการตรวจร่างกายและผลทางพยาธิวิทยาของต่อมน้ำเหลืองที่ขาหนีบรวมถึงผลพยาธิวิทยาของต่อมน้ำเหลืองที่เชิงกรานจะถูกนำมาเปรียบเทียบเพื่อหาความสัมพันธ์<br /><strong>ผลการศึกษา:</strong> ผู้ป่วย 19 รายที่เข้าเกณฑ์การคัดเข้า มี 14 รายที่เป็นกลุ่มความเสี่ยงสูง และ 5 รายเป็นกลุ่มความเสี่ยงต่ำอายุเฉลี่ยของผู้ป่วย 53 ปี การเก็บข้อมูลแยกแต่ละข้าง (1 คนจะมีรายงาน 2 ข้าง) การตรวจต่อมน้ำเหลืองบริเวณขาหนีบ พบตั้งแต่คลำ�ไม่ได้ จนถึงขนาดใหญ่ 6 ซม. จากการตรวจร่างกายพบการแพร่กระจายมาที่ต่อมน้ำเหลืองเชิงกราน5 ข้าง โดยเกณฑ์ขนาดต่อมน้ำเหลืองที่ขาหนีบน้อยกว่า 2 ซม. จะพบการกระจายมาที่เชิงกรานเพียง 1 ข้าง และจะมีโอกาสไม่พบการแพร่กระจายร้อยละ 97 ส่วนผลทางพยาธิวิทยาของต่อมน้ำ�เหลืองที่ขาหนีบนั้น พบจำ�นวนการแพร่กระจายมากที่สุด 9 ต่อม ถ้าใช้เกณฑ์จำนวนต่อมน้ำเหลืองที่น้อยกว่า 2 ต่อม พบว่ามีการแพร่กระจายไปต่อมน้ำเหลืองที่เชิงกรานเพียง 1 ข้าง และจะมีโอกาสไม่พบการแพร่กระจาย ร้อยละ 96<br /><strong>สรุป:</strong> การศึกษานี้พบว่า ผู้ป่วยที่คลำต่อมน้ำเหลืองที่ขาหนีบได้ไม่เกิน 2 ซม. มีโอกาสน้อยที่จะมีการแพร่กระจายไปเชิงกรานอย่างไรก็ตามยังต้องการการศึกษาที่มีจำนวนผู้ป่วยมากเพียงพอที่จะบ่งชี้ว่าการเลาะต่อมน้ำเหลืองไม่มีความจำเป็นในผู้ป่วยกลุ่มนี้ ER -
