Abstract

Factor Influencing the Treatment of Tuberculosis Patients in Saimai District, Bangkok Metropolitan Administration

Objective: To determine the results and factors influencing the treatment of tuberculosis in Saimai District, Bangkok Metropolitan Administration.

Study design: Descriptive and Qualitative study.

Subjects: A total of seventy one newly diagnosed tuberculosis patients in Public Health Center 61, Health Department and 2 general hospitals in Saimai District during January 2005 to May 2006 were enrolled.

Methods: Data from tuberculosis report form and medical records were reviewed and analyzed. Patients' behaviors toward treatment were evaluated by face-to-face interview.

Main outcome measures: Results of treatment, patients' behavior, compliance and satisfaction during the treatment.

Results: The cure rate of new tuberculosis patients with positive sputum examination in public health center 61 that providing the new line of treatment according to national tuberculosis program and the two hospitals providing outpatient management were 93.3% and 56.3% respectively.  Cure rate in patients with non-HIV infection was higher than in patients with HIV infection. Patients with good behavior and compliance during treatment were 87.5%.  The cure rate included complete treatment in this group was 97.6% while the patients who have to improve their behaviors in treatment and poor compliance in receiving the treatment had defaultion 5 in 6 cases.  Patients who was good satisfied in receiving the treatment was 70.8% and had cure and complete rate 97.1% while the patients who was modulate satisfied was 29.2% that had cure and complete treatment 10 in 14 cases.

Conclusion: The cure rate of tuberculosis treatment in public health center and hospitals were 93.3%  and 56.3% respectively.  The factor mostly influencing tuberculosis treatment was the new line of treatment according to the national tuberculosis program.  Other important factors were HIV infection, behaviors and satisfaction in receiving the treatment.
