Epignathus is a congenital oropharyngeal teratoma which is very rare. This report presented a case of epignathus in a fetus of a 38 years old woman at 23 weeks of gestation. The tumor was detected during an antenatal ultrasonography which revealed polyhydramnios, placenta previa totalis and a fetus with solid cystic mass sized 10 centimeters, filling the oropharyngeal cavity and protruding from the mouth. Termination of pregnancy was achieved by hysterotomy. A male fetus weighing 855 grams and 28 centimeters long was delivered. The postmortem pathologic examination confirmed the diagnosis of epignathus with midline defect over hard palate.
