ผลของตัวชี้นำด้วยแสงเลเซอร์ทางสายตาต่อการเดินติดและความยาวก้าวแรกในผู้ป่วยพาร์กินสัน

บทคัดย่อ
ปัจจุบันมีผู้ป่วยพาร์กินสันทั่วโลกเพิ่มมากขึ้น พบว่าปัญหาด้านการเดินติด (Freezing of gait, FOG) และความยาวก้าวแรก (First step length) ที่ลดลงในช่วงการเริ่มต้นการเดิน (Gait initiation) เป็นปัจจัยที่ทำให้มีการหกล้มได้ง่าย คุณภาพการใช้ชีวิตประจำวันลดลง มีการช่วยเหลือตนเองได้ยากลำบากขึ้น จนนำไปสู่การติดเตียง และทุพพลลภาพในอนาคต ดังนั้นการนำตัวชี้นำภายนอก (External cues) เข้ามาเป็นทางเลือกในการฝึกเดิน จึงเข้ามามีบทบาท โดยจะทำหน้าที่เป็นตัวกระตุ้น และเป็นเป้าหมายให้กับผู้ป่วยในระหว่างการก้าวเดิน ซึ่งตัวชี้นำภายนอก (External cues) ที่นิยมนำมาใช้กันคือ ตัวชี้นำทางด้านการมองเห็น (Visual cue) ในการศึกษานี้จะทำการศึกษาผลของตัวชี้นำด้วยแสงเลเซอร์ทางสายตาต่อการเดินติด และความยาวก้าวแรกในผู้ป่วยพาร์กินสัน
วัตถุประสงค์: เพื่อศึกษาผลของตัวชี้นำด้วยแสงเลเซอร์ทางสายตาต่อการเดินติด และความยาวก้าวแรกในผู้ป่วยพาร์กินสัน
วิธีดำเนินการวิจัย: อาสาสมัครผู้ป่วยโรคพาร์กินสัน จำนวน 11 คน อายุ 40–85 ปี มีระดับ Modified Hoehn and Yahr Scale อยู่ที่ 2 ถึง 3 ทำการจับฉลาก โดยแบ่งเป็น 2 รูปแบบ ดังนี้ ไม่มีตัวชี้นำ (No cue) และตัวชี้นำทางการมองเห็นที่เป็นแสงเลเซอร์ (Laser light) จากนั้นกลุ่มตัวอย่างดำเนินการทดสอบโดยทำการติดแสงเลเซอร์ไว้บริเวณด้านหน้ากึ่งกลางของกระดูกสันหลังส่วนล่างข้อที่ 5 (L5) ในรูปแบบของตัวชี้นำทางการมองเห็นที่เป็นแสงเลเซอร์ (Laser light) ทำการเก็บข้อมูล ได้แก่ ร้อยละของเวลาในการเกิดการเดินติด (Percent time spent freezing) และความยาวก้าวแรก (The first step length) แบ่งเป็น ความยาวก้าวแรกด้านซ้าย (Left first step length) และความยาวก้าวแรกด้านขวา (Right first step length) นำข้อมูลที่ได้มาวิเคราะห์ค่าเฉลี่ย (Mean) และส่วนเบี่ยงเบนมาตรฐาน (Standard deviation) และเปรียบเทียบความแตกต่างระหว่างรูปแบบ โดยใช้สถิติ Paired – Sample T Test ที่ระดับนัยสำคัญทางสถิติที่ 0.05
ผลการวิจัย: การเปรียบเทียบร้อยละของเวลาในการเกิดการเดินติด (Percent time spent freezing) และความยาวก้าวแรก (The first step length) แบ่งเป็น ความยาวก้าวแรกด้านซ้าย (Left first step length) และความยาวก้าวแรกด้านขวา (Right first step length) ระหว่างรูปแบบไม่มีตัวชี้นำ (No cue) และรูปแบบตัวชี้นำทางการมองเห็นที่เป็นแสงเลเซอร์ (Laser light) มีความแตกต่างกันอย่างมีนัยสำคัญที่ระดับ 0.05
สรุปผลการวิจัย: การใช้ตัวชี้นำทางการมองเห็นที่เป็นแสงเลเซอร์ (Laser light) สามารถลดปัญหาการเดินติด (Freezing of gait, FOG) และช่วยเพิ่มความยาวก้าวแรก (The first step length) ให้กับผู้ป่วยพาร์กินสัน
