ผลของการฝึกด้วยแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนของเลือดต่อองค์ประกอบของร่างกายในผู้สูงอายุ

บทคัดย่อ
บทนำ
ผู้สูงอายุมีการเสื่อมถอยของระบบต่างๆ ในร่างกายตามวัย โดยเฉพาะการสูญเสียมวลกล้ามเนื้อ การลดลงของอัตราการเผาผลาญพลังงาน และการเพิ่มขึ้นของมวลไขมัน ซึ่งสัมพันธ์กับความเสี่ยงของการเกิดโรคไม่ติดต่อเรื้อรัง งานวิจัยที่ผ่านมาพบว่าการออกกำลังกายแบบมีแรงต้านช่วยพัฒนาองค์ประกอบของร่างกายได้อย่างมีประสิทธิภาพ แต่การฝึกด้วยความหนักสูงอาจเพิ่มความเสี่ยงต่อการบาดเจ็บในผู้สูงอายุได้ ดังนั้นการฝึกด้วยแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนเลือด (Blood Flow Restriction: BFR) สามารถกระตุ้นการสร้างกล้ามเนื้อได้แม้ใช้แรงต้านระดับต่ำ ทั้งนี้งานวิจัยในผู้สูงอายุยังมีจำกัด การศึกษานี้จึงมุ่งหาแนวทางที่ปลอดภัยและมีประสิทธิภาพในการพัฒนาองค์ประกอบของร่างกายในผู้สูงอายุ
วัตถุประสงค์
การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของการฝึกด้วยแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนของเลือดต่อองค์ประกอบของร่างกายในผู้สูงอายุ
วิธีดำเนินการวิจัย
กลุ่มตัวอย่างเป็นผู้สูงอายุ อายุเฉลี่ย 65.92±3.67 ปี จำนวน 32 คน แบ่งเป็น 2 กลุ่ม ได้แก่ กลุ่มควบคุม (CON, n=16) และกลุ่มฝึกด้วยแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนเลือด (FT-BFR, n=16) โดยกลุ่มควบคุมดำเนินกิจวัตรประจำวันปกติ และกลุ่ม FT-BFR ทำการฝึกแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนเลือด เป็นเวลา 45 นาที 3 วันต่อสัปดาห์ ระยะเวลาการฝึก 8 สัปดาห์ และกำหนดความตึงของสายรัดยางยืดที่ร้อยละ 40 ของความดันปิดกั้นหลอดเลือดแดง ผู้วิจัยทำการทดสอบตัวแปรด้าน องค์ประกอบร่างกาย และสรีรวิทยาทั่วไปของร่างกาย ในช่วงก่อนการทดลอง และหลังการทดลองครบ 8 สัปดาห์ และนำข้อมูลมาวิเคราะห์ข้อมูลทางสถิติใช้แบบจำลองอิทธิพลผสม (Mixed-effects models) เพื่อเปรียบเทียบความแตกต่างของตัวแปรระหว่างก่อนและหลังการทดลอง รวมถึงทดสอบปฏิกิริยาร่วม (Interaction) ระหว่างเวลาและกลุ่มทดลอง และใช้สถิติทดสอบค่าทีสำหรับกลุ่มตัวอย่างที่เป็นอิสระต่อกัน (Independent Samples t-test) ในการเปรียบเทียบร้อยละความเปลี่ยนแปลงระหว่างกลุ่ม นอกจากนี้ ได้ทำการวิเคราะห์ขนาดอิทธิพล (Effect size; ES) เพื่อประเมินขนาดความแตกต่างของตัวแปร
ผลการวิจัย
พบว่าหลังจาก 8 สัปดาห์ กลุ่ม FT-BFR มีค่าเฉลี่ยของมวลปราศจากไขมัน (2.08 ± 2.22 , p=0.027) เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ ค่าเปอร์เซ็นต์ไขมัน (-1.92 ± 2.92 , p=0.018) และอัตราการเต้นของหัวใจขณะพัก (71.12 ± 4.96 , p=0.013) ลดลงอย่างมีนัยสำคัญทางสถิติเมื่อเทียบกับกลุ่มควบคุม (CON)
สรุปผลการวิจัย
การฝึกด้วยแรงต้านแบบฟังก์ชันร่วมกับการจำกัดการไหลเวียนของเลือดเป็นการฝึกที่ปลอดภัยและสามารถพัฒนาองค์ประกอบของร่างกาย ได้อย่างมีประสิทธิภาพในผู้สูงอายุ และอาจเป็นทางเลือกหนึ่งในการออกกำลังกายสำหรับประชากรกลุ่มนี้
