Patient inadequate decontamination in sodium hydroxide exposure: A case report

Abstract

   Sodium hydroxide, known as “Caustic soda”, is an alkali and highly corrosive chemical. Commonly using in industry as solid form and aqueous solution in various uses, including neutralization of acid; the manufacture of paper, textiles, plastics, dyestuffs, and corrosives; refining of petroleum; metal cleaning, etc. Exposure to sodium hydroxide can cause severe burn and deep tissue injury which may lead to permanent disability or death.

   This case report described clinical presentation of a worker who worked in automotive parts plating factory and accidentally fell in the tank that containing sodium hydroxide solution during his work. The patient was exposed to sodium hydroxide at the whole of body without wearing any personal protective equipment. He was decontaminated within 5 minutes with improper technique and inadequate rinsing time. He did not take off his underwear during decontamination so he had suffered from sodium hydroxide exposure by severe burn at his scrotum more than he should. Even though, he had been taken to the hospital and got adequate decontamination there just 45 minutes after this accident. So an adequate decontamination should be more important than how soon to get to hospital unless the patient had been found any significant symptoms that required emergency care.

   โซเดียมไฮดรอกไซด์หรือโซดาไฟมีคุณสมบัติเป็นด่างและมีฤทธิ์กัดกร่อน มักถูกใช้ในอุตสาหกรรมทั้งในรูปของแข็งและสารละลายในน้ำ โดยนำมาเป็นตัวทำปฏิกริยากับกรดให้เป็นกลาง เช่น ในอุตสาหกรรมกระดาษ เครื่องนุ่งห่ม พลาสติก สี สบู่ และสารกัดกร่อน นอกจากนั้น ยังนิยมใช้ในอุตสาหกรรมกลั่นปิโตรเคมี และการทำความสะอาดโลหะอีกด้วย หากสัมผัสโซเดียมไฮดรอกไซด์อาจก่อให้เกิดอันตรายต่อร่างกายโดยจะทำให้เกิดแผลไหม้อย่างรุนแรง และทำให้เยื่อบุต่างๆ ได้รับความเสียหาย ซึ่งนำไปสู่ภาวะทุพพลภาพถาวรหรือถึงแก่ชีวิตได้
   รายงานผู้ป่วยฉบับนี้บรรยายถึงอาการทางคลินิกของพนักงานคนหนึ่งในโรงงานชุบเคลือบชิ้นส่วนรถยนต์ที่เกิดอุบัติเหตุพลัดตกลงในบ่อบรรจุสารละลายที่มีส่วนผสมของสารโซเดียมไฮดรอกไซด์ระหว่างทำงาน จึงสัมผัสกับโซเดียมไฮดรอกไซด์ทั่วทั้งตัวโดยไม่ได้สวมอุปกรณ์ป้องกันส่วนบุคคลใดๆ ผู้ป่วยรายนี้ได้รับการล้างตัวเพื่อลดการปนเปื้อนจากการสัมผัสภายในห้านาที ด้วยวิธีการที่ไม่เหมาะสม และใช้เวลาล้างตัวและตาไม่นานพอ อีกทั้งยังไม่ได้ถอดกางเกงในออกระหว่างล้างตัว จึงได้รับผลกระทบจากการสัมผัสโซเดียมไฮดรอกไซด์โดยเกิดแผลไหม้บริเวณถุงอัณฑะรุนแรงกว่าที่ควรจะเป็น ทั้งที่ได้รับการรักษาและล้างตัวเพื่อลดการปนเปื้อนอีกครั้งอย่างถูกวิธีที่โรงพยาบาลภายหลังการสัมผัสเพียงสี่สิบห้านาที การลดการปนเปื้อนที่เพียงพอจึงน่าจะสำคัญกว่าความรวดเร็วในการนำผู้ป่วยส่งโรงพยาบาลหากผู้ป่วยไม่มีอาการผิดปรกติที่จำเป็นต้องได้รับการรักษาอย่างฉุกเฉิน
