Inadequate self-care can cause hospital re-admission. The effective self-care consists of educating patients on self-care maintenance, self-care monitoring and self-care management. Nurses should encourage the patients to gain confidence and maintain correct and consistent self-care.  Therefore, nurses who work at ambulatory care have the crucial role in caring for patients with chronic heart failure. These patients need continuing care after discharge from the hospital.
