ประสบการณ์การตั้งครรภ์ของมารดาอายุมากในอำเภอเมืองอุตรดิตถ์: การวิจัยเชิงคุณภาพแบบพรรณนา

บทนำ: การตั้งครรภ์ของมารดาอายุมาก (≥35 ปี) มีแนวโน้มเพิ่มขึ้นอย่างต่อเนื่องทั่วโลก แต่ประสบการณ์เชิงอัตวิสัยด้านการดูแลตนเอง การสนับสนุนทางสังคม และการเข้าถึงบริการสุขภาพในบริบทไทยยังมีข้อจำกัดทางความรู้อยู่มาก
วัตถุประสงค์: เพื่อสำรวจและบรรยายประสบการณ์การตั้งครรภ์ของมารดาอายุมากในอำเภอเมืองอุตรดิตถ์
วิธีการดำเนินการวิจัย: การวิจัยเชิงคุณภาพแบบพรรณนาครั้งนี้เก็บรวบรวมข้อมูลด้วยการสัมภาษณ์เชิงลึกกับผู้ให้ข้อมูล 18 ราย คัดเลือกด้วยวิธีเจาะจงร่วมกับการประสานงานผ่านอาสาสมัครสาธารณสุขประจำหมู่บ้านในฐานะผู้นำทาง วิเคราะห์ข้อมูลด้วยวิธี Thematic analysis ตามแนวทาง 6 ขั้นตอนของ Braun และ Clarke
ผลการวิจัย: ผลการวิจัยพบ 4 Themes หลัก ได้แก่ (1) ความรู้สึกสองขั้วระหว่างความดีใจและความกลัว: ประสบการณ์การตั้งครรภ์ในมารดาอายุมาก ผู้ให้ข้อมูลทุกรายมีความยินดีปนความวิตกกังวลต่อความผิดปกติของโครโมโซมซึ่งคลี่คลายลงได้บางส่วนหลังได้รับผลตรวจคัดกรองที่แสดงความเสี่ยงต่ำ (2) การดูแลตนเองอย่างมุ่งมั่นท่ามกลางความเหนื่อยล้าของวัย มีแรงจูงใจสูงด้านโภชนาการแต่จำกัดด้านการออกกำลังกาย (3) เครือข่ายสนับสนุนที่มีสามีเป็นแกนกลางร่วมกับบทบาทของอาสาสมัครสาธารณสุขประจำหมู่บ้านในฐานะผู้เชื่อมโยงระหว่างมารดากับระบบสุขภาพ และ (4) การเข้าถึงบริการฝากครรภ์: ทางเลือก อุปสรรค และความคาดหวังต่อระบบซึ่งอุปสรรคหลักคือการรอคิวนานและความเหนื่อยล้าทางกาย แม้ผู้ให้ข้อมูลส่วนใหญ่จะพึงพอใจต่อทัศนคติของบุคลากรสุขภาพ
สรุปผลและข้อเสนอแนะ: ผลการวิจัยชี้ให้เห็นความจำเป็นในการพัฒนาคลินิกฝากครรภ์เฉพาะสำหรับมารดาอายุมาก การเสริมสมรรถนะอาสาสมัครสาธารณสุขประจำหมู่บ้าน และการส่งเสริมการมีส่วนร่วมของสามีในกระบวนการฝากครรภ์อย่างเป็นระบบ
