ประสบการณ์ของสมาชิกครอบครัวในการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง

บทคัดย่อ

การวิจัยเชิงคุณภาพนี้ มีวัตถุประสงค์เพื่อบรรยายประสบการณ์ของสมาชิกครอบครัวในการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง ผู้ให้ข้อมูลเป็นสมาชิกครอบครัวที่มีประสบการณ์การดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรังที่เลือกแบบเฉพาะเจาะจงตามคุณสมบัติที่กำหนด จำนวน 10 ราย เก็บข้อมูลด้วยวิธีการสัมภาษณ์เชิงลึก การสังเกต และการจดบันทึกภาคสนาม วิเคราะห์ข้อมูลเชิงใจความหลัก

ผลการวิจัยพบว่า ประสบการณ์ของสมาชิกครอบครัวในการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง สามารถจำแนกได้เป็น 4 ประเด็นหลัก ได้แก่ 1) ความรู้สึกของสมาชิกครอบครัวต่อการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง ซึ่งแยกเป็น 2 ประเด็นย่อย คือ ความผูกพัน และเป็นหน้าที่ รวมถึงความรู้สึกเหนื่อยแต่ภูมิใจ 2) การดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรังซึ่งแยกเป็น 3 ประเด็นย่อย คือ การดูแลแบบทันใจ การดูแลเฉพาะโรค และการดูแลทางใจ 3) ปัญหาที่เกิดขึ้นจากการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง ซึ่งแยกเป็น 3 ประเด็นย่อย คือ การขาดรายได้ การขาดความเป็นอิสระ และการขาดความรู้ในการดูแล 4) การจัดการปัญหาของสมาชิกครอบครัวในการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง ซึ่งแยกเป็น 3 ประเด็นย่อย คือ การทำใจ การแสวงหาความรู้ และการช่วยเหลือจากญาติ

ผลการศึกษาครั้งนี้ พยาบาลสามารถนำมาเป็นข้อมูลพื้นฐานในการพัฒนารูปแบบเพื่อการสนับสนุนสมาชิกครอบครัวให้มีความพร้อมในการดูแลผู้ป่วยภาวะหัวใจล้มเหลวเรื้อรัง

Experiences of Family Members in caring of Patients with Chronic Congestive Heart Failure

The aim of this qualitative research was to describe the experiences of family members in caring of patients with chronic congestive heart failure. Ten informants were purposively selected according to inclusion criteria. In-depth interviews, observation, and field notes were used for data collection. Thematic analysis was employed to analyze the data.

The study results revealed that the experiences of family members in caring for patients can be divided into four themes: First, the feelings of family members towards caring for patients. This can be divided into two categories including bonding and duty, and feeling tired but proud. Second, the caring of patients, this theme was divided into three categories including fast care, special for the disease, and mental care. Third, the problems with patient care, this was divided into three categories including lack of income, lack of freedom, and lack of knowledge. Fourth, problem management of family members associated with caring for patients; this theme was divided into three categories including let it be, pursuit of knowledge, and help from the relatives.

Nurse can use this findings to be baseline data for the developing the model for enhancing family readiness in caring of patients with chronic congestive heart failure.
