กระบวนการปกป้องสิทธิผู้ป่วยของพยาบาลวิชาชีพ

บทคัดย่อ

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพแบบทฤษฎี
พื้นฐาน โดยมีวัตถุประสงค์เพื่อศึกษากระบวนการปกป้อง
สิทธิผู้ป่วยของพยาบาลวิชาชีพ ผู้ให้ข้อมูลเป็นพยาบาล
วิชาชีพที่ปฏิบัติการพยาบาลในโรงพยาบาลของจังหวัด
ระยองทั้งหมดจำนวน 20 คน เก็บรวบรวมข้อมูลโดยการ
สัมภาษณ์เชิงลึก การสังเกต และการวิเคราะห์บันทึก
ทางการพยาบาล วิเคราะห์ข้อมูลตามแนวคิดของ Strauss and Corbin (1998)

ผลการศึกษาพบว่า กระบวนการปกป้องของ
พยาบาลวิชาชีพเกิดขึ้นเป็นลำดับต่อเนื่อง คือ ระยะที่ 1
ทำตามและเรียนรู้ พยาบาลวิชาชีพเริ่มเข้าสู่วิชาชีพการ
พยาบาลให้การดูแลผู้ป่วยตามสิทธิพื้นฐานและการเรียนรู้
การปกป้องสิทธิผู้ป่วยจากพยาบาลวิชาชีพที่มีประสบการณ์
ระยะที่ 2 อึดอัด-ขัดแย้ง พยาบาลวิชาชีพถึงแม้มี
ประสบการณ์มากขึ้นแต่เมื่อไม่สามารถปกป้องสิทธิผู้ป่วย
ได้ เกิดความรู้สึกไม่สบายใจ ลำบากใจ เป็นห่วงผู้ป่วย ส่วน
ระยะที่ 3 ทุ่มเทอย่างสุดความสามารถ พยาบาลวิชาชีพ
พยายามอย่างสุดความสามารถโดยพยายามจัดการด้วย
ตนเองใช้พลังของทีม และผสมผสานกลยุทธ์ที่หลากหลาย
เพื่อปกป้องผู้ป่วย นอกจากนี้ยังพบว่า เงื่อนไขที่สำคัญได้แก่
การรับรู้เกี่ยวกับการปกป้องสิทธิผู้ป่วย การมีความ
เชี่ยวชาญเชิงวิชาชีพ การมีพลังในตนเอง และมีแหล่ง
สนับสนุนช่วยให้พยาบาลวิชาชีพปกป้องผู้ป่วยได้ ซึ่งทำให้
เกิดผลที่ตามมา คือ ผู้ป่วยได้รับการดูแลตามสิทธิพื้นฐาน

รักษาสิทธิให้กับตนเองได้
ผลการวิจัยที่ได้สามารถนำมาใช้เป็นข้อมูลพื้นฐาน
ในการพัฒนาความสามารถการปกป้องสิทธิผู้ป่วยของ
พยาบาลวิชาชีพ นอกจากนี้ยังช่วยให้ผู้บริหารทางการ
พยาบาลสามารถนำเงื่อนไขที่เกี่ยวข้องมาใช้สนับสนุน
พยาบาลวิชาชีพในกระบวนการปกป้องสิทธิผู้ป่วย ซึ่งส่งผล
ให้เกิดประโยชน์สูงสุดแก่ผู้ป่วย

The purpose of this grounded theory study
was to describe the process of advocating
patients’ rights of professional nurses. This study
was conducted in 20 professional nurses, who
worked in hospital at Rayong province. In-depth
interview, observation, and analyze nursing records
were the strategies used in data collection. Strauss
and Corbin’s (1998) approach was used to analyze
the data.
The result showed that the process of
advocating patients’ rights of professional
nurses composes of three sequential phases; 1)
Following and learning is the early phase for the
beginner nurses. They just do following basic
patients’ rights and learn from experience nurses.
2) The feeling of uncomforting and conflict;

professional nurses gain more experience. They
feel uncomfortable when they still are unable to
advocate patients’ rights. 3) Dedicating to the
best own ability is the last phase; professional
nurses see something that are not right or wrong
morally, they just take actions by themselves,
or uses power of team and multi-strategies to
uphold to advocate patients’ rights. Perception,
professional expert, self-power, and support were
conditions to advocate patients’ rights. The
consequence of this process was that the patients
receive the standard treatments safely, including
realizing their rights and protecting themselves.
The finding of this study can be used
as the basis to develop competencies of
professional nurses to advocate patients’ rights.
In addition, nursing administrator can use
conditions to support professional nurses to
advocate patients’ rights. This will promote the
best benefit of the patients.
