การพัฒนารูปแบบการบริหารจัดการเชิงยุทธศาสตร์เพื่อยกระดับประสิทธิภาพการดำเนินงานกองทุนหลักประกันสุขภาพระดับท้องถิ่นหรือพื้นที่ของหน่วยบริการสาธารณสุขในจังหวัดเพชรบุรี

การวิจัยครั้งนี้เป็นการวิจัยในรูปแบบวิจัยและพัฒนา (Research and Development) มีวัตถุประสงค์เพื่อศึกษาสภาพการดำเนินงานและปัญหาอุปสรรคของกองทุนหลักประกันสุขภาพระดับท้องถิ่นหรือพื้นที่ (กปท.) พัฒนาและประเมินความเหมาะสมของรูปแบบการบริหารจัดการเชิงยุทธศาสตร์ การวิจัยแบ่งเป็น 3 ระยะ ได้แก่ การสำรวจสภาพปัจจุบัน การพัฒนารูปแบบ และการประเมินรูปแบบ โดยเก็บข้อมูลเชิงปริมาณจากบุคลากรสาธารณสุข 342 คน เก็บข้อมูลเชิงคุณภาพจากผู้ให้ข้อมูลสำคัญ 18 คน สนทนากลุ่มผู้ปฏิบัติงาน 10 คน และให้ผู้ทรงคุณวุฒิประเมินรูปแบบฯ ที่พัฒนาขึ้น 12 คน และเวทีประชาพิจารณ์ 123 คน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ค่าสัมประสิทธิ์สหสัมพันธ์ภายในกลุ่ม และการวิเคราะห์เนื้อหา
ผลการวิจัยพบว่าสภาพการดำเนินงานและประสิทธิภาพการดำเนินงานอยู่ในระดับมาก ข้อมูลเชิงคุณภาพสะท้อนปัญหาเชิงระบบ ได้แก่ การขาดการมีส่วนร่วมของชุมชน การมุ่งเน้นความครบถ้วนของเอกสารมากกว่าผลลัพธ์เชิงสุขภาพ ความซับซ้อนของขั้นตอน และการขาดกลไกพี่เลี้ยงสนับสนุนในระดับอำเภอ รูปแบบที่พัฒนาคือ “Initiate–P–D–C–A” บูรณาการกับ POLC ประกอบด้วย การวางแผนแบบใช้ข้อมูล มาตรฐานงาน การบริหารกำลังคน ระบบการเงินโปร่งใส ภาวะผู้นำ การติดตามประเมินผล และความยืดหยุ่นของการเข้าถึง ผลการประเมินพบว่ารูปแบบมีความเหมาะสมในระดับมาก (M= 3.84, SD= 0.11) โดยมีความเห็นพ้องต้องกันในระดับดี (ICC = 0.75) สรุปได้ว่า Initiate–P–D–C–A เป็นกลไกเตรียมความพร้อมเชิงระบบที่สำคัญต่อการยกระดับประสิทธิภาพการดำเนินงาน กปท. ในจังหวัดเพชรบุรี
