ความสัมพันธ์ของฟันผุกับภาวะโภชนาการในเด็กไทยอายุ 5 ปี

บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ของฟันผุในฟันน้ำนมกับภาวะโภชนาการของเด็กไทยอายุ 5 ปี โดยใช้ข้อมูลการสำรวจสภาวะทันตสุขภาพแห่งชาติครั้งที่ 7 พ.ศ.2555 สุ่มจาก 4 ภาค ภาคละ 4 จังหวัด และกรุงเทพมหานคร รวม 17 จังหวัด การตรวจฟันผุใช้เกณฑ์รูผุขององค์การอนามัยโลก วัดเป็นรายซี่ การประเมินภาวะโภชนาการใช้น้ำหนักและส่วนสูงตามเกณฑ์อายุของสำนักโภชนาการ กรมอนามัย ดำเนินการระหว่าง ธันวาคม 2558 ถึง มีนาคม 2559 ผลการศึกษา เด็ก 2,626 คนมีฟันผุร้อยละ 78.5 โดยผุ 1-4 ซี่ ร้อยละ 35.8 และผุมากกว่า 4 ซี่ ร้อยละ 42.7 สภาวะโภชนาการเมื่อวัดโดยน้ำหนักเทียบกับอายุ พบเด็กมีน้ำหนักน้อยกว่าเกณฑ์ ตามเกณฑ์ และ มากกว่าเกณฑ์ ร้อยละ 19.6, 70.4 และ 10.0 ตามลำดับ เมื่อวัดโดยเกณฑ์ส่วนสูงเทียบกับอายุ พบเด็กเตี้ยกว่าเกณฑ์ สูงตามเกณฑ์ และสูงกว่าเกณฑ์ ร้อยละ 13.0, 78.8 และ 3.4 ตามลำดับ เด็กที่มีฟันผุมากกว่า 4 ซี่ มีค่าเฉลี่ยของน้ำหนักและส่วนสูง น้อยกว่าเด็กที่มีฟันผุ 1-4 ซี่ หรือเด็กที่ไม่มีฟันผุ อย่างมีนัยสำคัญทางสถิติ (p<.001) เด็กที่มีฟันผุมากกว่า 4 ซี่ มีโอกาสเป็นเด็กน้ำหนักน้อยกว่าเกณฑ์ 1.4 เท่าของเด็กที่ไม่มีฟันผุ (OR=1.4, 95 % CI 1.1-1.9) และ มีโอกาสเตี้ยกว่าเกณฑ์ 1.5 เท่าของเด็กที่ไม่มีฟันผุ (OR=1.5 95 % CI 1.1-2.0)
