ความเหลื่อมล้ำในพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียนในพื้นที่เทศบาลตำบลบางปู จังหวัดสมุทรปราการ

บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาความเหลื่อมล้ำในพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำและปัจจัยที่มีผลต่อพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียนในเทศบาลตำบลบางปู ดำเนินการเก็บข้อมูลจากผู้ปกครองโดยใช้แบบสอบถาม ผลการศึกษาพบว่ามีกลุ่มตัวอย่างเพียงร้อยละ 18.3 ที่มีพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำ การศึกษาพบความเหลื่อมล้ำของพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียน โดยพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียนนั้นกระจุกตัวอยู่ในกลุ่มผู้ปกครองที่มีการศึกษาสูง ประกอบอาชีพชั้นสูง มีรายได้ครอบครัวสูง และมีเศรษฐานะระดับสูง และจากผลการวิเคราะห์การถดถอยโลจิสติค พบปัจจัยที่ส่งผลต่อพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียน คือ สถานภาพทางเศรษฐกิจและสังคมของผู้ปกครอง ผู้ปกครองที่มีเศรษฐานะระดับกลางชั้นสูงและระดับสูงจะมีบุตรหลานที่มีพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำมากกว่ากลุ่มระดับล่างชั้นสูง (OR=2.13; 95% CI=1.05-4.31) และผู้ปกครองมีพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำจะมีแนวโน้มที่บุตรหลานมีพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำมากกว่ากลุ่มผู้ปกครองที่ไม่มีพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำ (OR=4.11; 95% CI=2.66-6.60) โดยสรุปพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำของเด็กก่อนวัยเรียนสัมพันธ์กับสถานภาพทางเศรษฐกิจและสังคมของผู้ปกครอง เพื่อลดความเหลื่อมล้ำในพฤติกรรมการตรวจสุขภาพช่องปากเป็นประจำ การออกแบบนโยบายให้มีประสิทธิภาพและตรงกลุ่มเป้าหมายโดยเฉพาะกลุ่มที่มีเศรษฐานะต่ำ ควรมีการนำข้อมูลจากหลักฐานทางวิชาการรวมถึงข้อมูลประชากรในพื้นที่มาช่วยในการวางยุทธศาสตร์ของนโยบายเพื่อเพิ่มการเข้าถึงบริการทางทันตกรรม นอกจากนี้การพัฒนาศักยภาพของระบบบริการให้สามารถตอบสนองความต้องการและวิถีชีวิตของประชาชนก็เป็นสิ่งที่ผู้กำหนดนโยบายสุขภาพในพื้นที่ควรให้ความสำคัญ
