ประเมินผลโครงการฟันเทียมพระราชทานจังหวัดนนทบุรี พ.ศ. 2558 - 2562

บทคัดย่อ
การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อประเมินผลโครงการฟันเทียมพระราชทานปี 2558 - 2562 โดยใช้ CIPP model จากทันตแพทย์ 29 คน และผู้รับบริการ 287 คน พบการจัดบริการภาพรวมทำได้ตามเป้าหมาย ปัจจัยนำเข้า กำลังคน งบประมาณ วัสดุอยู่ในระดับมาก ( = 3.9) การจัดบริการส่วนมากทำในโรงพยาบาล การมีส่วนร่วมของเจ้าพนักงานทันตสาธารณสุขในพื้นที่และองค์กรปกครองส่วนท้องถิ่น อยู่ในระดับน้อย ( =2.3) ระยะเวลารอคิวทำฟันเทียม 3-10 เดือน ด้านผู้รับบริการพบร้อยละ 26.1 กลับมาตรวจหลังใส่ฟันเทียม เมื่อสอบถามเกี่ยวกับสภาพฟันเทียม พบฟันเทียมแตกบิ่น ร้อยละ 8.7 ฟันเทียมบนหลวมร้อยละ 18.5 ต้องการปรับแต่งหรือทำฟันเทียมใหม่ ร้อยละ 57.5 กลุ่มที่รู้สึกว่าฟันเทียมบนหลวม แตกหัก หรือใช้ฟันเทียมมานานกว่า 5 ปี ต้องการเปลี่ยนฟันเทียมมากขึ้นอย่างมีนัยสำคัญทางสถิติ (p <0.05)
การดูแลฟันเทียมทำได้เหมาะสม โดยร้อยละ 82.1 แปรงฟันเทียม ร้อยละ 59.3 ล้างด้วยน้ำ ร้อยละ 25.3 ใช้ยาเม็ดทำความสะอาด การรับทราบสิทธิ์ในการทำฟันเทียมใหม่เพียงร้อยละ 22 ทราบว่าสามารถใช้สิทธิ์ทำฟันเทียมใหม่ได้เมื่อฟันเทียม มีอายุ 5 ปี การประเมินคุณภาพชีวิตในมิติสุขภาพช่องปากไม่พบความสัมพันธ์ระหว่างอายุกับ Oral Impacts on Daily Performances (OIDP) ข้อเสนอเพื่อพัฒนา 1. กระจายบริการไปที่โรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) เพื่อลดปัญหาการเดินทางของผู้สูงอายุ 2. เพิ่มการมีส่วนร่วมขององค์กรปกครองส่วนท้องถิ่นเพื่อร่วมจัดพาหนะรับส่งผู้ป่วย ให้อาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) และเจ้าพนักงานทันตสาธารณสุขในพื้นที่ ประชาสัมพันธ์ นัดหมาย เตรียมช่องปากและอนาคตอาจให้เอกชนร่วมจัดบริการ 3. เพิ่มความสำคัญในสิทธิ์การรับบริการแก้ไขฟันเทียมหลังใช้งาน และสิทธิ์ในการทำฟันเทียมชุดใหม่ จัดระบบอำนวยความสะดวกในการรับบริการแก้ไขฟันเทียมและติดตามดูแลการใช้งานและสภาพของฟันเทียมของผู้ป่วยอย่างต่อเนื่อง
