บทคัดย่อ
การวิจัยนี้เป็นการศึกษาแบบภาคตัดขวาง มีวัตถุประสงค์เพื่อประเมินผลโครงการฟันเทียม รากฟันเทียมเฉลิมพระเกียรติฯ ในเขตสุขภาพที่ 10 (ตุลาคม 2565 - กันยายน 2567) โดยใช้ CIPP model เพื่อประเมินการขับเคลื่อนโครงการ ผลการดำเนินงานและคุณภาพชีวิตของผู้รับบริการ ในเขต 10 ครอบคลุมจังหวัด อุบลราชธานี ศรีสะเกษ ยโสธร อำนาจเจริญ และมุกดาหาร วิธีการคือทบทวนเอกสาร เก็บข้อมูลระหว่าง ระหว่าง มกราคม - มีนาคม 2567 ในกลุ่มตัวอย่าง 3 กลุ่ม คือผู้บริหารจากสำนักงานสาธารณสุขจังหวัด จำนวน 5 คน ผู้ปฏิบัติเป็นทันตแพทย์หัวหน้ากลุ่ม และทันตแพทย์ที่รับผิดชอบโครงการจำนวน 23 คน คัดเลือกแบบเจาะจง ผู้รับบริการเป็นผู้สูงอายุที่รับบริการใส่ฟันเทียมรองรับรากฟันเทียมในโครงการ ที่ได้รับบริการอย่างน้อย 6 เดือนและยินดีให้ข้อมูล จำนวน 56 คน ข้อมูลเชิงคุณภาพใช้วิเคราะห์ข้อมูลเชิงเนื้อหา (Content analysis) และข้อมูลเชิงปริมาณใช้สถิติ Wilcoxon signed-rank test เปรียบเทียบคะแนนเฉลี่ยคุณภาพชีวิตในมิติสุขภาพช่องปาก ที่นัยสำคัญ 0.05 ด้านบริบท (Context) มีการจัดสรรหน้าที่และกระจายเป้าหมายในหน่วยบริการรากฟันเทียม จำนวน 23 แห่ง ด้านปัจจัยนำเข้า (Input) มีการรับนโยบายทุกจังหวัด (ร้อยละ 100.0) มีการพัฒนาบุคลากรเพื่อฝังรากฟันเทียม สิทธิการรักษาฟันเทียมทำได้ทุกสิทธิ รากฟันเทียมตามสิทธิหลักประกันสุขภาพแห่งชาติ สนับสนุนนวัตกรรมรากฟันเทียมที่ใช้วัสดุไทเทเนียมเกรด 4 ด้านกระบวนการ (Process) ฟันเทียมมีการตั้งรับผู้รับบริการโดยคัดกรองตามเกณฑ์ งานรากฟันเทียมค้นหาเชิงรุกนอกพื้นที่เพื่อให้ความรู้และตรวจคัดกรอง ด้านผลผลิต (Output) ในไตรมาสที่ 4 (30 กันยายน 2567) มีการฝังรากฟันเทียมและฟันเทียม ร้อยละ 100 ตามเป้าหมาย พบปัจจัยความสำเร็จเกิดจากการวางแผน การมีส่วนร่วม และการประยุกต์ใช้เทคโนโลยี และผลการประเมินคุณภาพชีวิตในมิติสุขภาพช่องปากผู้รับบริการดีขึ้นทั้ง 7 ด้าน ได้แก่ 1) รับประทานอาหาร 2) พูด 3) ทำความสะอาดช่องปาก 4) รักษาอารมณ์ 5) ยิ้ม หัวเราะโดยไม่อาย 6) พบปะผู้คน 7) การทำงาน และกิจวัตรประจำวัน (p-value < 0.05) โดยความพึงพอใจภาพรวมทุกด้านอยู่ในระดับมากที่สุด สรุปได้ว่าผู้รับบริการมีคุณภาพชีวิตในมิติช่องปากดีขึ้น จากการดำเนินงานเชิงรุกและรับที่มีประสิทธิภาพ จึงเสนอให้มีการขยายสิทธิรากฟันเทียมให้ใช้ได้ทุกสิทธิเพื่อความครอบคลุม
