Abstract

การปลูกถ่ายตับเป็นการรักษาขั้นสุดท้าย ในการรักษาแบบครบวงจรสำหรับผู้ป่วยที่มีโรคตับเรื้อรังระยะสุดท้ายซึ่งไม่สามารถใช้วิธีการอื่นใดมาช่วยให้ตับทำหน้าที่ให้เหมาะสมเพียงพอต่อความต้องการของร่างกาย การผ่าตัดเช่นนี้มีความละเอียดอ่อนอย่างมาก ต้องการการดูแลเป็นพิเศษหลายๆ ด้าน ทั้งด้านความรู้ ความสามารถ ทักษะในการผ่าตัด การวางยาสลบในภาวะที่ตับเสื่อมสภาพ การดูแลเรื่องการช่วยหายใจ ปฏิกิริยาภูมิคุ้มกัน โภชนาการ การดูแลปัญหาติดเชื้อ การประเมินสภาพของหลอดเลือดหลังผ่าตัดโดยวิธีการด้าน imaging ทั้งด้วยรังสีหรือสารกัมมันตรังสี หรือคลื่นเสียงความถี่สูงและอื่น ๆ อีกมาก โดยเฉพาะในผู้ป่วยเด็กไม่ว่าจะในแง่ของด้านการแพทย์ที่พบ ปัญหาอื่นร่วมกันกับผู้ป่วยประเภทนี้ เช่น ภาวะทุโภชนาการ พัฒนาการช้า ภาวะเลือดออกง่าย นอกเหนือจากปัญหาตับวาย เรื้อรัง รวมทั้งในแง่จิตวิทยาของทั้งเด็กและครอบครัวของเด็ก
