Descending thoracic aortic aneurysm

Abstract

               การรักษา descending thoracic aortic aneurysm ด้วยวิธีการทางศัลยกรรมนี้ เพิ่งมีวิวัฒนาการมาได้ไม่ถึง 50 ปี โดยในปี 1951 Gross et al รายงานถึงความสำเร็จใน การทำ replacement of descending aortic aneurysm ด้วย aortic homograft และในปีเดียวกันนั้นเอง Lam and Aram รายงานถึงการทำ replacement of descending aortic aneurysm ด้วย aortic homograft แต่พบว่า ผู้ป่วยเกิด postoperative paraplegia, empyema และ เสียชีวิตหลังผ่าตัด 6 อาทิตย์ ต่อมาในปี 1953 Bahnson รายงานถึงความสำเร็จในการ repair saccular aneurysm ของ thoracic aorta ด้วยวิธี lateral resection and aortorrhaphy และในปีเดียวกันนั้นเอง DeBakey and Colley ก็ได้รายงานถึงความสำเร็จในการ replacement descending thoracic aortic aneurysm ด้วย prosthetic จนถึงปัจจุบันนี้การผ่าตัด descending thoracic aortic aneurysm ได้มีวิวัฒนาการมากขึ้น มีเทคนิคและวัสดุอุปกรณ์ มากขึ้น แต่ก็ยังพบว่ามีข้อขัดแย้งอยู่โดยเฉพาะเกี่ยวกับการใช้ เทคนิค bypass หรือ shunt เพื่อนำเลือดไปเลี้ยงส่วนล่าง ของร่างกาย
