ผลการศึกษาระยะยาวของ High tibial osteotomyในโรงพยาบาลพระจอมเกล้า

บทคัดย่อ

วัตถุประสงค์  :  ประเมินผลในระยะยาวของ ผู้ป่วยข้อเข่าเสื่อมที่มีขาโก่ง ที่ทำ high tibial osteotomy และวิเคราะห์ ปัจจัยที่เกี่ยวข้องกับผลในระยะยาว


วัสดุและวิธีการ  :  การติดตามผลแบบไปข้างหน้าของ การผ่าตัด ข้อเข่าเสื่อมที่มีขาโก่ง จำนวน 39 เข่า จากผู้ป่วย 39 ราย ที่ทำ closed wedge high tibial osteotomy ระหว่างปี พ.ศ. 2539 ถึง พ.ศ. 2547 ระยะเวลาของการติดตามเฉลี่ย 10.12 ปี  การประเมินผลทางคลินิกโดยใช้  knee score ของ American Knee Society


ผลการศึกษา  :  ผลที่เวลา  5 ปี หลังการผ่าตัดและ การติดตามครั้งสุดท้าย ได้ผลเป็นที่พอใจ 89.73% และ 53.84% ตามลำดับ พบว่าผู้ป่วยที่มีดัชนีมวลร่างกายสูง ( BMI ≥ 25 kg/m2) และมี knee flexion contracture ≥15ºจะมีผลที่ไม่น่าพอใจในการติดตามผลในระยะยาว  นอกจากนี้การคืนรูปของขาโก่ง จะเกิดขึ้นหลัง 5 ปี ของการผ่าตัด มากกว่าในระยะแรกของการติดตาม


สรุป  :  ในผู้ป่วยข้อเข่าเสื่อมที่มีขาโก่ง และยังต้องทำงานหนัก การทำ High tibial osteotomy ยังเป็นทางเลือกที่เหมาะสม โดยที่ผู้ป่วยอ้วนที่มี  BMI ≥ 25 kg/m2 หรือ ข้อเข่ามี flexion contracture  ≥ 15º เป็นปัจจัยสำคัญต่อผลในระยะยาว
