การสำรวจความรู้ ทัศนคติ และแนวปฏิบัติเกี่ยวกับโรคมะเร็งช่องปาก ในผู้เข้ารับการฝึกอบรมเพื่อวุฒิบัตรสาขาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียล ในประเทศไทย: การศึกษาแบบตัดขวาง

บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อสำ รวจความรู้ ทัศนคติ และแนวปฏิบัติในการตรวจวินิจฉัยและการดูแลรักษาโรคมะเร็งช่องปาก และหาปัจจัยที่เกี่ยวข้องกับความรู้ ทัศนคติ และแนวปฏิบัติดังกล่าวของผู้เข้ารับการฝึกอบรมเพื่อวุฒิบัตรสาขาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียลในประเทศไทย ที่ได้เริ่มเข้ารับการฝึกอบรมตั้งแต่ปี พ.ศ.2561ถึงปี พ.ศ.2564 ด้วยแบบสอบถามออนไลน์ชนิดตอบด้วยตนเอง มีผู้ตอบกลับแบบสอบถาม 73 ราย จากกลุ่มตัวอย่าง89 รายคิดเป็น 82.0% ผู้เข้ารับการฝึกอบรมมีคะแนนเฉลี่ยความรู้เกี่ยวกับโรคมะเร็งช่องปากในระดับปานกลาง คะแนนเฉลี่ยความรู้เกี่ยวกับโรคมะเร็งช่องปากของผู้ที่เข้ารับการฝึกอบรมชั้นปีที่ 3 และชั้นปีที่ 4 สูงกว่าผู้ที่เข้ารับการฝึกอบรมชั้นปีที่ 1 และชั้นปีที่ 2 อย่างมีนัยสำ คัญทางสถิติ (p = 0.007) ด้านทัศนคติพบว่า ผู้ที่อบรมในหลักสูตรสาขาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียลร่วมกับการศึกษาแพทยศาสตรบัณฑิตมีคะแนนเฉลี่ยทัศนคติที่มีต่อการดูแลรักษาโรคมะเร็งช่องปากสูงกว่าผู้ที่อบรมในหลักสูตรสาขาศัลยศาสตร์ช่องปากและแม็กซิลโลเฟเชียลอย่างมีนัยสำ คัญทางสถิติที่ระดับ0.05 (p = 0.003) ผู้เข้ารับการฝึกอบรม 69.9% มีการตรวจครบถ้วนทุกตำแหน่งในช่องปาก ทั้งนี้ยังพบว่า retromolar trigone เป็นตำแหน่งในช่องปากที่มักจะลืมตรวจมากที่สุดคิดเป็น 16.4% ผู้เข้ารับการฝึกอบรมที่มีประสบการณ์ในการ ตรวจผู้ป่วยมะเร็งช่องปากรายใหม่ที่เข้ารับการรักษาและผู้ป่วยรายเก่าที่มาติดตามภายหลังการรักษามากกว่า 5 ราย/ปี ทำการคลำ ต่อมนํ้าเหลืองบริเวณคอบ่อยขึ้นอย่างมีนัยสำ คัญทางสถิติ (p = 0.032) อาจสรุปผลได้ว่า จำ นวนปีที่เข้ามา รับการฝึกอบรมมีผลต่อความรู้เกี่ยวกับโรคมะเร็งช่องปากของผู้เข้ารับการฝึกอบรม ผู้เข้ารับการฝึกอบรมในหลักสูตร ที่แตกต่างกันอาจมีทัศนคติต่อการดูแลรักษาโรคมะเร็งช่องปากที่แตกต่างกันไป ทั้งนี้ยังพบว่า ผู้เข้ารับการฝึกอบรมมี การลืมตรวจบางตำแหน่งในช่องปาก แต่ภาพรวมแล้วแนวปฏิบัติเกี่ยวกับการตรวจวินิจฉัยและการดูแลรักษาโรคมะเร็ง ช่องปากของผู้เข้ารับการฝึกอบรมเป็นไปในทางที่เหมาะสม
