Insulin and mortality in hospitalized pulmonary tuberculosis with diabetes mellitus: a retrospective cohort study

Abstract

OBJECTIVE

To identify the relationship between using insulin and mortality in patients with pulmonary tuberculosis with diabetes mellitus.

METHODS

We performed a retrospective cohort study to assess the association between using insulin and mortality in hospitalized patients with tuberculosis with diabetes mellitus in Khon Kaen Hospital, Thailand. Data were collected from December 2009 to November 2012. Insulin injection was our exposure of interest regardless of dosage and types. Our primary outcome was mortality.

RESULTS

From December 2009 to November 2012, there were 152 hospitalized patients with pulmonary tuberculosis with diabetes mellitus. They were divided into two groups; 102 cases used insulin mono-therapy and 50 cases used oral antihyperglycemic agents with or without insulin therapy. We found that insulin mono-therapy was associated with higher mortality rate (hazard ratio, 4.00; 95% confidence interval, 1.81 to 8.85).

CONCLUSION

Among hospitalized patients with pulmonary tuberculosis with diabetes mellitus, insulin mono-therapy was associated with higher mortality rate.
