Improvement of a Continuing Care Model of the Home-bound and Bed-bound Stroke’s Patients in Nonghan Hospital Udon Thani Province

Abstract

This study is to develop the quality of Primary Health Care by applying a participatory action research process. The duration of the study in January 2019 to October 2019 was carried out into 3 phases, namely 1) Assessment phase 2) The development phase of the model, which is considered as the operation of 3 operations cycles and, 3) Evaluation phase. Participants consisted of 1) 30 family members of stroke’s patients who voluntarily participated in the study 2) 15 Village Health Volunteers (VHV) from 15 places, 3) 15 Registered Nurse at Health Promoting Hospital 4) 3 persons responsible for chronic disease at Nonghan Hospital and 5) A multidisciplinary team consisting of physician, pharmacist, nutritionist, public health academic, and Thai traditional medicine total of 5 people. The researcher has collected data and facilitated the discussion and brainstorming sessions. The statistics used in the analysis of quantitative data by frequency distribution, percentage, while the qualitative data used content analysis and the participants were involved in the analysis in every process. The results of the research showed that;

All 39 the Home-bound and Bed-bound Stroke’s Patients were 23 males (58.97%) and 16 females (41.03%) and they were evaluated by Activities of Daily Living (ADL) for assessment functional independence namely, 15 persons (43.60%) total dependence, 17 persons (43.60%) severe dependence. Besides, participants were appraised for losing the ability in activities of daily living which found out the 32 samples (82.05%) are most impairment the ability of daily living and the 7 subjects (17.95%) are more decline in the ability of daily living

Continuing Care Model of the Home-bound and Bed-bound Stroke’s Patients in Nonghan Hospital Udon try design out, applied concepts, exchanging experiences, reflections and developed the model as follows: 1) Obvious model and mutual awareness 2) Rules for working with multidisciplinary teams and networks 3) Able to solve problems and provide comprehensive care for stroke patients.
