Abstract

The objective of this action research was to develop a model for promoting healthy behaviors of uncontrolled diabetic patients by applying community participation program. The purposive samples were conducted in 15 uncontrolled diabetic patients.  Questionnaires: demographic and health care behavior data; focus group; observation; sound recorder; and a glucose meter were used to collect the data between November 2019 and January 2020.  The research was conducted into two phases. Firstly, preparation phase which the preliminary health conditions of the patients were studied.  Secondly, research phase which consisted of four steps: 1) presenting of current problems, setting desired goals and planning, 2) discussing appropriate and prioritizing interventions, 3) outcome observation and approach, and 4) monitoring and evaluation of the results.   

The study found that the overall health care behaviors of the participants were at a moderate level, with a mean of 2.78 (S.D. = 0.42).  There were significantly different in blood glucose levels before and after community participation (p < 0.05).  This study concluded that by sharing knowledge among diabetic patients, families, caregivers and community participation among community leaders, health volunteers and health care providers, a joint solution was conducted.  It was notable that community participation also enhance the participants’ empowerment and encouragement to solve blood sugar uncontrolled problems.  Problem solving was done according to the problem conditions and community and family contexts.  This resulted in increased awareness of blood sugar control among diabetic patients.
