Abstract

The purpose of this research is developed of continuing care guidelines for dependent patients by using the process of stakeholder participation. There are two phases of the research.The participants for development phase were 10 dependent patients and caregivers, 17 health personals, and 3 health volunteers, the data were collected by focus group discussions and in-depth interviews. Analyze data with content analysis. The evaluation period for the appropriateness of the continuation of care approach with 25participants whoinvolved in continuation of care. The instrument was a 5 rating scale with Cronbach's alphaconfidence level of 0.81). Data analyzed by mean and standard deviation.

The results showed that Guidelines consist of 1) assessment of the needs of patients and caregivers. By using the framework for the distribution plan of DMETHOD 2) Recording information in the Home Care Program3) Assessing the readiness of patients and caregivers.4) prepare knowledge, and prepare the necessary equipment

5) Coordinate a multidisciplinary team for practice caring skills as needed. 6) Continuous care plan 7) Record data in the Smart COC program 8) Follow up on continuous care at home from the Smart COC program 9) Summarize and analyze continuous care data according to the outcome goals. Both quantitative and qualitative10) Report the performance to the hospital quality development committee.

The evaluation of the appropriateness of the continuous care method is generally the most appropriate, with an average value of 4.79 (0.22). The average number of activities of continuous care at home is 4.90 (0.50)
