Developmental of nursing management model for Prevention and Control of antimicrobial-resistant bacteria Transmission Sakon Nakhon Hospital

This action research aimed to analyze the situation, develop the nursing model and model evaluation for prevention and control of antimicrobial-resistant bacteria transmission in Sakon Nakhon hospital. The research was implemented from October, 2018 to September, 2020. The sample three groups, 1) team to analyze the situation, develop and evaluate the nursing model 30 persons 2) nurse 955 persons 3) patients admitted to hospital during the study period. The research tools were semi-structured interview form, supervision plan, practice observation form, record form for antimicrobial-resistant bacteria surveillance and knowledge test.
The results found that the developed model consisted of 13 activities; 1) nursing care of antimicrobial-resistant bacteria infected patients when receiving services 2) practice guidelines for prevention and control of
antimicrobial-resistant bacteria transmission 3) isolation precaution 4) wearing personnel protective equipment 5)hand hygiene 6) disinfection 7) Follow-up antimicrobial-resistant bacteria infected 8) patient transfer and refer 9) assignment 10) supervision 11) equipment support 12) nursing documentation and 13) infection surveillance and alerts. The evaluation after model development showed that the knowledge scores of nurse after model development were higher than before model development (p< .05). The proportion of correct practices of registered nurse was increased (p< .05). The incidence of antimicrobial-resistant bacteria infection was decreased from 1.63 to 1.4 per 1,000 patient-days. Antimicrobial cost was decreased from 25,095.65 to 17,011.20 baht/person (p< .05). The length of stay and mortality rate were not change significantly.
