รูปแบบการป้องกันการพลัดตกหกล้มซ้ำของผู้สูงอายุในโรงพยาบาล: การทบทวนวรรณกรรมแบบบูรณาการ

การป้องกันการพลัดตกหกล้มซ้ำของผู้สูงอายุในโรงพยาบาลที่มีประสิทธิภาพ จะช่วยลดอุบัติการณ์การพลัดตกหกล้มซ้ำ และยกระดับคุณภาพชีวิตของผู้สูงอายุอย่างยั่งยืน การศึกษานี้มีวัตถุประสงค์เพื่อวิเคราะห์และสังเคราะห์องค์ความรู้เกี่ยวกับองค์ประกอบ วิธีการดำเนินการ และผลลัพธ์ของรูปแบบ โปรแกรม หรือแนวทางการป้องกันการพลัดตกหกล้มซ้ำในผู้สูงอายุที่เข้ารับการรักษาในโรงพยาบาล มีรูปแบบและวิธีการวิจัยเป็นการทบทวนวรรณกรรมแบบบูรณาการ โดยใช้กรอบแนวคิด PICO/PICo ในการคัดเลือกงานวิจัย ครอบคลุมการศึกษาเชิงพรรณนา เชิงสำรวจ เชิงคุณภาพ และเชิงทดลอง ดำเนินการตามแนวทางของ Whittemore and Knafl (2005) ประกอบด้วย 5 ขั้นตอน ได้แก่ การกำหนดปัญหา การค้นหาและคัดเลือกข้อมูล การประเมินคุณภาพ การวิเคราะห์สังเคราะห์ข้อมูล และการนำเสนอผลการทบทวน จากฐานข้อมูล MEDLINE Complete, CINAHL Complete, ThaiJO และ Google Scholar ในช่วงปี พ.ศ. 2559–2569 รวมทั้งสิ้น 1,119 เรื่อง คัดเลือกเข้าสู่การวิเคราะห์จำนวน 11 เรื่อง โดยประเมินคุณภาพด้วยเกณฑ์ของ Joanna Briggs Institute (2022) มีการตรวจสอบความถูกต้อง การประเมินค่า การสกัดข้อมูลโดยผู้วิจัย และมีการใช้ thematic analysis และ narrative synthesis/summary ในการสังเคราะห์ข้อมูล
ผลการวิจัยพบว่า รูปแบบการป้องกันการพลัดตกหกล้มซ้ำของผู้สูงอายุในโรงพยาบาล มีลักษณะเป็นการจัดการหลายปัจจัย ประกอบด้วย การประเมินความเสี่ยง การดูแลเฉพาะราย การจัดการปัจจัยเสี่ยง การให้ความรู้และการมีส่วนร่วม การจัดการสิ่งแวดล้อม การทำงานร่วมกันของทีมสหสาขาวิชาชีพ และการดูแลต่อเนื่อง วิธีการดำเนินการมีความหลากหลายและควรปรับให้เหมาะสมกับระดับความเสี่ยง ความสามารถ และบริบทของผู้สูงอายุ โดยเชื่อมโยงการดูแลตั้งแต่โรงพยาบาลก่อนจำหน่ายจนถึงหลังจำหน่าย ผลลัพธ์ครอบคลุมการเกิดหกล้มและหกล้มซ้ำ ความเสี่ยงต่อการหกล้ม การทรงตัว สมรรถภาพทางกาย ความสามารถในการทำกิจวัตรประจำวัน และความกลัวการหกล้ม ทั้งนี้ ควรนำองค์ประกอบที่สังเคราะห์ได้ไปประยุกต์ใช้ตามบริบทมากกว่าการใช้รูปแบบมาตรฐานเดียว
ผลการวิจัยชี้ให้เห็นว่า การป้องกันการพลัดตกหกล้มซ้ำในผู้สูงอายุควรดำเนินการภายใต้แนวคิดการดูแลแบบองค์รวมและบูรณาการหลายปัจจัย โดยพยาบาลมีบทบาทสำคัญในการประเมิน วางแผน ประสานการดูแล และติดตามต่อเนื่องตั้งแต่โรงพยาบาลสู่บ้าน ควรนำองค์ประกอบที่สังเคราะห์ได้ไปพัฒนาและปรับใช้เป็นแนวทางการดูแลเฉพาะรายตามบริบทของหน่วยบริการ พร้อมส่งเสริมการติดตามหลังจำหน่าย และควรมีการศึกษาวิจัยเชิงทดลองเพื่อประเมินประสิทธิผลของแนวทางดังกล่าวอย่างเป็นระบบ
