Abstract

     This research aimed to develop a rehabilitation nursing care model for heart failure patients (HFs) in Pattani Hospital and to evaluate the outcomes of the nursing care model for rehabilitation implementation. A sample of 25 patients with heart failure and 56 registered nurses were recruited using a simple random sampling method. Research instruments included the rehabilitation nursing care model for heart failure patients, and questionnaires to gather data on demographic information, heart failure patients, caregiver, registered nurses’ interview, knowledge, ability to walk on the ground, re-admission, and nurses’ opinions. Descriptive statistics and Paired t-test were used to analyze the data.

      The results revealed that the rehabilitation nursing care model for heart failure patients at Pattani hospital consisted of nursing care in the acute phase, before and after discharge. After using the developed rehabilitation nursing model, the heart failure patients had a statistically significantly higher level of practical knowledge than before (p < .05), and the six-minute walk on the ground was statistically significantly greater than before (p < .05). Within 28 days, none of the heart failure patients were readmitted (100%). The findings suggest that nurses can apply this model while caring for coronary heart disease patients in other hospitals. This will further increase the patients’ knowledge and ability to self-care, thus reducing re-admission and healthcare costs.
