Development of a model for slowing down kidney deterioration in patients with Chronic Kidney Disease at Damnoensaduak Hospital, Ratchaburi Province

The purposes of this research and development study were to 1) develop and study the results of using a model for slowing down kidney deterioration in patients with Chronic Kidney Disease. The sample was divided into 2 groups, including 24 and 355 customers. The research instruments were divided into 2 parts: the instruments used to collect data were the model for slowing down kidney deterioration, and the evaluation tools for service provider groups included a knowledge assessment form, a nursing practice questionnaire, the feasibility of using the model, and the instruments for customer groups, including a knowledge assessment form, a practice to slow down kidney deterioration questionnaire, and a satisfaction assessment. The data were analyzed using descriptive statistics and t-tests. The results revealed as follows:
1. Models for slowing down kidney deterioration in patients with chronic kidney disease include planning, organizing support structures, promoting compliance with standards, data management, and linkage to continuing care
2. After using the model, it was found that blood pressure levels, blood sugar and HbA1c were statistically significantly lower than before using the model at .01 level, and the glomerular filtration rate was statistically significantly better than before using the model at the .01 level.
Therefore, an effective model for delaying kidney deterioration should promote the potential of patients and their families in terms of knowledge, practice, and continuous care to adjust behavior in an appropriate and sustainable way
