Abstract

The objectives of this case study was to study and analyzed nursing care for patients with acute ischemic stroke who received Thrombolytic Agen drug (rt-PA). Selecting patients with complicated by obstructing of the large cerebral arteries and therefore receiving intravenous blood clot dissolving drugs. This requires joint care with a multidisciplinary team. The nurses have an important role in closely caring for patients and must integrate nursing concepts, theories, and 5-step nursing process, including assessment, diagnosis, planning implementation, and evaluation. In planning nursing care for the patients in 3 phases, including nursing care during an initial period, nursing care during the intensive care period, and nursing care during subacute rehabilitation. The result of nursing care found that the patient was in the critical period and was the risk of intracranial pressure from obstructing the large cerebral arteries therefore receiving Thrombolytic Agen drug (rt -PA), and closely observing symptoms was no bleeding in the brain, and began to the recovery period. The nurses reinforce in rehabilitation and health care of patients in every aspect, emphasizing physical therapy. occupational therapy, planning follow -up for continuous physical recovery, and forwarding information to plan home health care with the multidisciplinary team.
