Development of Service System in Operation Room to Prevent Surgery on the Wrong Person, Wrong Organ, and Wrong Position

This participatory action research aimed to develop and evaluate the appropriateness of a nursing service system in operating rooms to prevent wrong-patient, wrong-site, and wrong-procedure surgeries. The study included 45 multidisciplinary professionals working in the operating room, who participated from August 2022 to August 2023. The research was conducted in four phases: planning, action, evaluation/observation, and reflection.
The research instruments included: 1) guidelines for preventing wrong-patient, wrong-site, and wrong-procedure surgeries in operating rooms, 2) a surgical safety checklist, and 3) a satisfaction questionnaire. The Index of Consistency of the research instruments ranged from 0.84 to 1.00. The inter-rater reliability of the surgical safety checklist was 0.91, and the Cronbach's alpha coefficient of the satisfaction questionnaire was 0.98. Data was analyzed using descriptive statistics. The findings revealed that:
The developed guidelines for preventing wrong-patient, wrong-site, and wrong-procedure surgeries were accurate, conformed to the standards of the World Health Organization, and covered all phases of surgical procedures. The participants reported the highest level of satisfaction with the guidelines. Furthermore, no adverse incidents were reported during the implementation of the guidelines. It is recommended that these collaboratively developed guidelines be adopted to enhance patient safety outcomes in surgical services.
