Comprehensive Health Care for Older Adults in Primary Health Care Systems: The Roles of Community Nurse Practitioners and Integrated Care

Abstract

Health care for older adults is a crucial issue in supporting healthy aging societies. Community nurse practitioners play a significant role in driving the provision of comprehensive primary health care services for older adults within communities. The primary goal is to deliver holistic care in collaboration with multidisciplinary teams, encompassing health promotion, disease prevention, medical treatment, and physical rehabilitation, as well as health care across all stages of illness, including emergency care, intermediate care, long-term care, rehabilitation, and palliative end-of-life care.

The implementation process integrates the World Health Organization’s primary health care framework, integrated care for older people, and comprehensive geriatric assessment into health service delivery. Key activities include health screening, individualized care planning, health promotion programs, continuous home visits, and the application of innovations and technologies in health care management. Major challenges include the integration of digital technologies to enhance the efficiency of health service delivery, the development of specialized competencies among health personnel, and the promotion of equitable access to health care services.

Therefore, comprehensive primary health care services for older adults should be aligned with the living contexts of service recipients, emphasize person-centered care, promote equitable access to health services, and concurrently strengthen workforce competencies. These approaches are considered essential strategies for advancing primary health care systems toward a healthy aging society.
