Abstract

This participatory action research aimed to: 1) study the situations of problem osteoarthritis in elderly, 2) Development of Caring System for Elderly with Knee Osteoarthritis by Community Participation and 3) evaluate and study Effects of Caring System for Elderly with Knee Osteoarthritis by Community Participation and comprised three phases. Phase I: situational analysis. Qualitative data were also collected from 30 participants through in-depth interviews and 30 participants through focus group discussions, Phase II: development of Caring System for Elderly with Knee Osteoarthritis by Community Participation that consisted, in which 20 personnel identified planned interventions, implemented activities, and monitored and refined the processes. Phase III: evaluate the effectiveness of Caring System for Elderly with Knee Osteoarthritis by Community Participation. The sample groups were 60 pairs of village health volunteers and Elderly with Knee Osteoarthritis. They were divided into two groups, control group (n = 30) and experimental group (n = 30). The questionnaire about knowledge with the reliability of .82, practical skills questionnaire replied with the reliability of .80, and Used tools comprised severity assessment form for knee osteoarthritis (Oxford Knee Score) were collected. Data was analyzed content analysis for qualitative data. using percentage, mean and standard deviation for quantitative data. The mean score of knowledge, practical skills and Used tools comprised severity assessment form for knee osteoarthritis (Oxford Knee Score) between the experimental group and the control group while independent t-test and Paired t-test

The findings revealed that A community-based care system for older adults with knee osteoarthritis encompasses five key domains: 1) Health service system for elderly people with osteoarthritis 2 ) Equipment and environmental, 3) Assistive Devices & Environment, 3) social and psychological support systems, 4) Hearth promotion and prevention systems, and 5) health information and monitoring systems, The results development of Caring System revealed that.1) after 12th week period, the experimental group had higher mean score of knowledge, , and practical skills in Caring for Elderly with Knee Osteoarthritis than the control group at significant level 0.05 (t = 40.390, p < 0.001, and t = 36.295, p <0.001) 3) Elderly people with osteoarthritis who received care from village health volunteers after 12 th week period had an average score of Oxford knee score higher than before receiving care at significant level 0.05 (t = 5.02, p < 0.001)

Development of Caring System for Elderly with Knee Osteoarthritis by Community Participation could well affect the health volunteers who had knowledge and skill for caring the elderly with Knee Osteoarthritis and elderly with Knee Osteoarthritis had lower severity of knee osteoarthritis.
