Factors Influencing Self-Care Behaviors to Delay Chronic Kidney Disease Progression Among Community Dwelling Patients with Diabetes Mellitus

Abstract

       This study aimed to evaluate self-care behaviors intended to delay kidney function decline and to identify the psychosocial factors influencing these behaviors among patients with diabetes mellitus. A purposive sample of 131 patients with diabetes mellitus, diagnosed with estimated glomerular filtration rate (eGFR) stages 2–4, was recruited from the Mueang District of Suphan Buri Province, Thailand. Data were collected utilizing four structured questionnaires: demographic profiles, targeted self-care behaviors, self-care knowledge, and perceived influencing factors. The instruments demonstrated robust content validity indices (CVI) of 0.96, 0.93, and 0.90, alongside high reliability coefficients of 0.835, 0.762, and 0.898, respectively. Data were analyzed using descriptive statistics, correlation coefficients, and multiple linear regression.

       The findings revealed that overall self-care behaviors among the participants were at a moderate level (M=2.99, SD=0.19). Significant positive correlations were observed between self-care behaviors and perceived self-efficacy, self-care knowledge, perceived severity, perceived susceptibility, and perceived benefits. The multiple regression analysis indicated that the proposed model accounted for 47.4% of the variance in self-care behaviors (Adjusted R²=.474, p<.001). Notably, perceived self-efficacy (β=.508, p<.001), self-care knowledge (β=.352, p<.001), and perceived benefits (β=.160, p<.05) emerged as significant independent predictors.

      To effectively delay the progression of diabetic nephropathy, a critical complication of diabetes, clinical interventions should strategically prioritize the enhancement of self-efficacy in tandem with comprehensive health education tailored to patients currently exhibiting moderate selfcare behaviors.
