Social Determinants of Health and Quality of Life Associated with Falls among Community-Dwelling Elderly in Suphan Buri, Thailand: A Cross-Sectional Study

This study aimed to examine the quality of life, social determinants of health, and falls among older adults, as well as to analyze factors influencing falls among older adults in Suphan Buri Province, Thailand. The sample consisted of 200 community-dwelling older adults aged 60 years and above, selected using a multi-stage sampling technique. The research instrument comprised a four-part questionnaire that was assessed for content validity, with Content Validity Index (CVI) values ranging from 0.90 to 0.97, and reliability coefficients ranging from 0.77 to 0.87. Data were collected using questionnaires and analyzed using descriptive statistics, including percentages, means, and standard deviations, as well as correlation analysis and multiple regression analysis.
The findings revealed that most older adults had a moderate level of quality of life (53.0%), while 47.0% had a good quality of life (M = 2.47, SD = 0.50). The regression analysis indicated that social determinants of health explained 24.3% of the variance in falls among older adults (Adjusted R² = 0.243), and the model was statistically significant (p < 0.001). Factors significantly associated with falls included income adequacy (b = -0.11, β = -0.29, p = 0.001) and accessibility to healthcare services, particularly the convenience of traveling to a physician or primary healthcare center (b = 0.15, β = 0.19, p = 0.019).
These findings suggest that older adults at greater risk of falls are those who remain physically active and are able to travel independently outside their homes, as reflected by easier access to healthcare services, but who continue to experience economic constraints, particularly inadequate income. Therefore, relevant agencies should integrate fall prevention strategies with the development of elderly-friendly community environments, particularly for low-income older adults, in order to sustainably reduce fall incidence and fall-related injuries among the elderly.
