Pancreatic panniculitis in male systemic lupus erythematosus

Abstract

Pancreatic panniculitis is a rare subtype of panniculitis which is associated with pancreatic disease. We report a case of a 34-year-old male with a history of systemic lupus erythematosus (SLE) complicated with proliferative lupus nephritis on maintenance azathioprine (AZA) who developed painful red nodules on the lower legs. He denied any symptom of pancreatitis. The histopathology showed subcutaneous lobular fat necrosis with anuclear adipocytes and granular deposits of calcium, consistent with pancreatic panniculitis. The investigations showed elevated lipase and amylase and the computed tomography of the abdomen revealed pancreatic pseudocyst. When AZA was discontinued in our patient, the level of lipase and amylase were decreased by half supporting the hypothesis of AZA-induced pancreatitis apart from the underlying SLE itself.
