การพัฒนารูปแบบเพิ่มพูนการใช้กระบวนการพยาบาล ในโรงพยาบาลบ้านบึง

Abstract

การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วมนี้มีวัตถุประสงค์ เพื่อพัฒนาและทดสอบประสิทธิผลของรูปแบบเพิ่มพูน การใช้กระบวนการพยาบาลของพยาบาลวิชาชีพ ในโรงพยาบาล บ้านบึง ผู้เข้าร่วมวิจัยเป็นผู้บริหารการพยาบาลและพยาบาล วิชาชีพที่ปฏิบัติงานในหอผู้ป่วยในโรงพยาบาลบ้านบึง กลุ่มตัวอย่างเป็นพยาบาลที่ปฏิบัติงานในหอผู้ป่วยใน จำนวน 22 คน และเวชระเบียนผู้ป่วยใน จำนวน 90 แฟ้ม เลือกโดยการสุ่ม เครื่องมือวิจัยเป็นแบบวัดความรู้การใช้ กระบวนการพยาบาลและแบบสอบถามทัศนคติต่อการใช้ กระบวนการพยาบาลที่ผู้วิจัยสร้างขึ้น และแบบประเมิน คุณภาพบันทึกทางการพยาบาลที่พัฒนาโดยกลุ่มการ พยาบาลโรงพยาบาลศูนย์ชลบุรี วิเคราะห์ข้อมูลโดยใช้สถิติ บรรยายและ t-test ผลการวิจัยมีดังนี้

1. รูปแบบเพิ่มพูนการใช้กระบวนการพยาบาล ของพยาบาลวิชาชีพ ในโรงพยาบาลบ้านบึงมี 7 องค์ ประกอบได้แก่ 1) การตั้งคณะทำงานผูรั้บผิดชอบการพัฒนา และปรับปรุงการใช้กระบวนการพยาบาลและบันทึกทางการ พยาบาล 2) การสร้างพยาบาลแกนนำในแต่ละหอผู้ป่วย ให้เป็นพี่เลี้ยงพยาบาลในการนำไปปฏิบัติ 3) การจัดประชุม เชิงปฏิบัติการเพื่อเพิ่มพูนความรู้และทัศนคติที่ดี 4) การประชุมคณะทำงานผู้รับผิดชอบการพัฒนาและปรับปรุง การใช้กระบวนการพยาบาลและพยาบาลแกนนำประจำ หอผู้ป่วย 5) การสื่อสารนโยบายการใช้กระบวนการพยาบาล และรูปแบบการบันทึกทางการพยาบาลสู่พยาบาลผู้ปฏิบัติงาน ที่เกี่ยวข้อง 6) การจัดทำแบบฟอร์มและคู่มือแนวทางการ ใช้กระบวนการพยาบาลและการบันทึกทางการพยาบาลและ 7) การจัดระบบติดตามและประเมินการใช้

2. ภายหลังการใช้รูปแบบฯ พยาบาลมีความรู้การ ใช้กระบวนการพยาบาลค่อนข้างอ่อน (M = 5.14, SD = 1.64) แต่มีทัศนคติต่อการใช้กระบวนการพยาบาลอยู่ในระดับดี (M = 4.37, SD = 0.35) และคุณภาพบันทึกทางการ พยาบาล สูงกว่าก่อนใช้ฯ อย่างมีนัยสำคัญทางสถิติที่ระดับ .001

Development of a model for enhancing the use of nursing process in Banbung Hospital.

The purposes of this participatory action research were to develop a model for enhancing the use of nursing process among nurses in Banbung hospital and to test its effectiveness. Research participants included nurse administrators and staff nurses in Banbung hospital. The samples of 22 staff nurses and 90 patient records were randomly selected. Research tools were a knowledge test, a questionnaire on attitude toward the use of nursing process and a checklist on the quality of nursing documentation. Descriptive statistics and t-test were used in data analysis. Research findings were as follows.

1. The model for enhancing the use of nursing process among nurses in Banbung hospital comprised of 7 components namely; 1) an appointment of the taskforce committee for improving the use of nursing process, 2) an appointment of the nurse core group for mentoring the use of nursing process in each ward, 3) a workshop arrangement on knowledge and attitude improvement, 4) a regularly meeting among the taskforce committee and the nurse core group, 5) transferring new policy regarding nursing process and documentation to all involved staff nurses, 6) development of new forms, handbooks and guidelines on using nursing process which include all aspects of health, and 7) set up a clear system for monitoring and evaluating the use of nursing process and the quality of nursing documentation.

2. After implementation of the model, staff nurses had knowledge on the use of nursing process at a poor level (M = 5.14, SD = 1.64) but had high positive attitudes towards the use of nursing process (M = 4.36, SD =0.35 ). The quality of nursing documentation was significantly higher than the preimplementation peroid at p < .001.
