The Effectiveness of Dental Pit and Fissure Sealant Program for the Student in Prathomsueksa 1 Cha-am District, Phetchaburi Province.

Abstract
The objective of this study was to evaluate the effectiveness of the dental pit and fissure sealant program for the students in prathomsueksa 1 Cha - am District by evaluation of the retention rate and the caries occurrence rate of permanent first molars of the students involved in the program and compared to the students not involved in the program after a period of 24 months. The samples were the students who had sound permanent first molars that should be sealed. The total number of the sample size was 398 persons, with 771 teeth, divided into 2 groups. The first group was 183 students, 349 teeth, while the other was the students not involved in the program, 215 students ,442 teeth. After 24 months of the program period, the samples involved in the program were examined to find out carious status and retention rate of sealant. The samples not involved in the program were examined to find out only the status of caries. Chi-square test was applied to analyze the data. The study found that the sealant conditions were complete retention 41.8%, partial retention 24.7% and complete loss33.5%, while the caries occurrence rate was 16.3%. The operators were significantly related to the complete retention rate (p = 0.001). There was no significant difference in complete retention rate between mandibular permanent first molar and maxillary permanent first molar. There was no significant difference in caries occurrence rate between mandibular permanent first molar and maxillary permanent first molar. There were no caries found in the teeth with complete retention of sealant. The teeth with complete loss had the chance to be caries 8.42 times to the teeth with partial loss (OR = 8.42, 95%CI= 3.59-19.65). The caries occurrence rate between the received sealant application group and, the not received sealant application group was not significantly different. To sum up, the dental pit and fissure sealant program was effective among the students in prathomsueksa 1. However, we still need to improve the quality of service and the service systems in order to reach the highest effectiveness of the program.
