บทคัดย่อ
การศึกษาเชิงพรรณนานี้มีวัตถุประสงค์เพื่อวิเคราะห์ความสมบูรณ์ในการบันทึกข้อมูลทันตกรรมในโปรแกรมฮอสเอ็กซ์พี โรงพยาบาลธวัชบุรี จากข้อมูลที่บันทึกระหว่าง 1 มีนาคม พ.ศ.2558 ถึง 28 กุมภาพันธ์ พ.ศ.2560 ตรวจสอบและประเมินความสมบูรณ์โดยทันตแพทย์คนเดียวตามเกณฑ์คู่มือการบันทึกข้อมูลทางทันตกรรมสำหรับโรงพยาบาลส่งเสริมสุขภาพตำบลในอำเภอธวัชบุรี หมวดที่มีข้อมูลที่ไม่สมบูรณ์มากที่สุด 3 อันดับแรกคือ ประวัติและการวินิจฉัย ตรวจทันตสุขภาพ และการรักษา (ร้อยละ 74.5, 18.0 และ 7.5 ตามลำดับ) ในหมวดประวัติและการวินิจฉัย เป็นการไม่บันทึกข้อมูลในรายการประวัติอื่นๆ สัญญาณชีพ และผลการวินิจฉัย (ในประเภทการวินิจฉัยหลัก) ร้อยละ 62.6, 23.2 และ 10.9 ตามลำดับ ในประวัติอื่นๆ มีรายการย่อยที่ไม่ได้บันทึกมากที่สุด 1 รายการ (ร้อยละ 74.0) โดยเป็นการไม่บันทึกประวัติทางทันตกรรมมากที่สุด (ร้อยละ 64.3) ขณะที่สัญญาณชีพมีรายการย่อยที่ไม่ได้บันทึกมากที่สุด 2 รายการได้แก่ อัตราการหายใจและอุณหภูมิร่างกาย (ร้อยละ 35.1) ความไม่สมบูรณ์ในหมวดตรวจทันตสุขภาพ 3 อันดับแรกคือ การไม่บันทึกรายการย่อยใดๆ เลยมากที่สุด (ร้อยละ 65.7) รองลงมาคือ ไม่บันทึกรายการ-ย่อย 1 รายการ (ร้อยละ 31.9 ในจำนวนนี้เป็นการไม่บันทึกบริการใน/นอกสถานที่มากถึงร้อยละ 29.1) และไม่บันทึกรายการย่อย 2 รายการ (ร้อยละ 0.2 ในจำนวนนี้เป็นการไม่บันทึกประเภทผู้ได้รับการตรวจทันตสุขภาพและบริการ ทันตกรรมใน/นอกโรงพยาบาลถึงร้อยละ 37.7) ความสมบูรณ์ของข้อมูลมีผลต่องบประมาณที่โรงพยาบาลได้รับจัดสรรตามผลงานจากสำนักงานหลักประกันสุขภาพแห่งชาติและการประเมินผลการดำเนินงานในการบริหารราชการทุกระดับ การสะท้อนกลับไปยังผู้บันทึกข้อมูลจึงจำเป็นเพื่อให้ตระหนักถึงความสำคัญโดยตรวจสอบและแก้ไข ก่อนส่งออกไปคลังข้อมูลสุขภาพส่วนกลาง
