บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนบริการทันตกรรมของโรงพยาบาลบ้านกรวดในปีงบประมาณ 2561 เป็นรายบริการ จากฐานข้อมูล 4 ชุดได้แก่ ระบบสารสนเทศสำหรับโรงพยาบาล ฎีกาการเบิกจ่ายเงินเดือนค่าตอบแทนและสวัสดิการ ระบบเบิกวัสดุ และทะเบียนครุภัณฑ์ วิเคราะห์ต้นทุนทางตรงรวมซึ่งประกอบด้วยต้นทุนค่าแรง ค่าวัสดุ และต้นทุนลงทุนซึ่งคิดจากค่าเสื่อมราคาครุภัณฑ์ทันตกรรมและอาคาร กระจายค่าแรงตามเวลาที่ใช้ในการบริการรายบริการ สำหรับต้นทุนวัสดุกระจายค่าวัสดุสิ้นเปลืองตามปริมาณที่ใช้รายบริการ วัสดุประเภทเครื่องมือซึ่งใช้งานได้หลายปีคิดเป็นมูลค่าต่อเวลาใช้งาน และรวมค่าจ้างทำฟันเทียมในภายหลัง ส่วนค่าเสื่อมราคาครุภัณฑ์กระจายตามอายุการใช้งานและจำนวนครั้งบริการขณะที่คิดค่าเสื่อมพื้นที่คลินิกทันตกรรมตามอายุการใช้งานอาคารและจำนวนการใช้บริการ พบว่า สัดส่วนต้นทุนค่าแรงต่อค่าวัสดุต่อค่าลงทุนเท่ากับ 72.7: 23.3: 4.0 บริการทันตกรรมประดิษฐ์มีต้นทุนต่อหน่วยสูงที่สุดและเคลือบหลุมร่องฟันต่ำที่สุดเท่ากับ 86.7 บาทต่อซี่ ต้นทุนบริการทุกงานต่ำกว่าอัตราค่าบริการของกระทรวงสาธารณสุข พ.ศ. 2560 และกรมบัญชีกลาง พ.ศ. 2559 ยกเว้นเดือยฟันชนิดเหวี่ยงมีต้นทุนสูงกว่าอัตราค่าบริการของกรมบัญชีกลาง
