ปัจจัยที่มีผลต่อการเข้ารับการตรวจสุขภาพช่องปากในผู้ใหญ่วัยทำงาน: ข้อมูลการสำรวจสภาวะสุขภาพช่องปาก ครั้งที่ 8 ประเทศไทย พ.ศ. 2560

บทคัดย่อ
การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อการพบทันตแพทย์เพื่อเข้ารับการตรวจสุขภาพช่องปากในรอบปีที่ผ่านมาของผู้ใหญ่วัยทำงาน โดยการวิเคราะห์ข้อมูลทุติยภูมิจากการสำรวจสภาวะสุขภาพช่องปากครั้งที่ 8 ประเทศไทย พ.ศ.2560 ตามกรอบแนวคิดซึ่งอ้างอิงตามแบบจำลองพฤติกรรมการใช้บริการสุขภาพของ Andersen ผลการศึกษาพบว่า จากกลุ่มตัวอย่างจำนวน 4,574 คน มีเพียงร้อยละ 6.6 ที่ไปพบทันตแพทย์เพื่อรับการตรวจสุขภาพช่องปากในรอบปีที่ผ่านมา การวิเคราะห์การถดถอยโลจิสติค พบว่าปัจจัยระดับบุคคลที่มีความสัมพันธ์กับการรับบริการตรวจสุขภาพช่องปาก ประกอบด้วย 1) ปัจจัยนำ ได้แก่ ระดับการศึกษา และพฤติกรรมการสูบบุหรี่ 2) ปัจจัยเอื้อ ได้แก่ รายได้เฉลี่ยต่อเดือนและสิทธิรักษาพยาบาล 3) ปัจจัยความต้องการด้านสุขภาพ ได้แก่ ความพึงพอใจในสุขภาพช่องปากของตน ด้านปัจจัยบริบทที่มีความสัมพันธ์กับการรับบริการตรวจสุขภาพช่องปาก ได้แก่ 1) เขตที่อยู่อาศัย 2) จำนวนทันตแพทย์ต่อประชากร 3) ความสะดวกของการคมนาคมและความครอบคลุมของการสื่อสารของจังหวัดที่กลุ่มตัวอย่างอาศัยอยู่ โดยกลุ่มตัวอย่างที่มีการศึกษาระดับสูง (OR=2.90; 95% CI= 1.55-5.42) รายได้เฉลี่ยระดับสูง (OR=3.22; 95% CI=1.10-9.45) และมีความพึงพอใจในสุขภาพช่องปากของตนระดับมาก (OR=3.02; 95% CI=2.06-4.42) เข้ารับการตรวจสุขภาพช่องปากมากกว่ากลุ่มที่มีระดับการศึกษา รายได้เฉลี่ยและความพึงพอใจในสุขภาพช่องปากของตนในระดับต่ำ ส่วนกลุ่มตัวอย่างที่สูบบุหรี่ (OR=0.49; 95% CI=0.31-0.79) และอาศัยอยู่ในเขตกรุงเทพมหานคร (OR=0.32; 95% CI=0.14-0.75) จะเข้ารับการตรวจสุขภาพช่องปากน้อยกว่ากลุ่มที่ไม่สูบบุหรี่และอาศัยนอกเขตเทศบาล การเข้ารับการตรวจสุขภาพช่องปากนั้นสัมพันธ์กับปัจจัยบุคคลและปัจจัยบริบท ดังนั้นการเพิ่มการเข้าถึงบริการสุขภาพช่องปากเพื่อการป้องกันจำเป็นต้องมีการพัฒนาโครงสร้างของระบบบริการอื่น ๆ ควบคู่ไปกับการขยายสิทธิการรักษา
