Abstract

Purpose: To study the influences of caregivers’ anxiety, depression, perceived adequacy of income, and patient’s dependence on the caregivers’ readiness for hospital discharge.


Design: A correlational predictive design.


Methods: The sample consisted of 108 caregivers of patients with first time stroke admitted in a tertiary care hospital in Bangkok.  Data were collected using the demographic questionnaire, a readiness questionnaire, a hospital anxiety and depression scale, and the patient’s dependence assessment scale.  Data were analyzed using descriptive statistics and stepwise multiple regression analysis.


Main findings: The participants had their readiness at a moderate level (Mean = 21.5, SD = 6.2), while many had low anxiety (64.8%), low depression (66.7%), almost half perceived adequacy of in come with savings (41.7%), and about one third of the patients had the highest dependence (34.3%).  The caregivers’ anxiety was able to explain 19% of variances in the caregivers’ readiness before discharge.  The caregivers’ anxiety was found to be the only factor influencing the caregivers’ readiness for hospital discharge in patients diagnosed with first time stroke with statistical significance (β = - .43, p < .05).


Conclusion and recommendations: Nurses should assess caregivers’ anxiety in order to make a care plan to provide support for them. Furthermore, nursing care should be developed to reduce caregivers’ anxiety in order to increase their readiness to care for the patients with first time stroke before hospital discharge.


 


บทคัดย่อ


        วัตถุประสงค์: เพื่อศึกษาอำนาจการทำนายของความวิตกกังวลของผู้ดูแล อาการซึมเศร้า การรับรู้ความเพียงพอของรายได้ และระดับการพึ่งพาของผู้ป่วย ต่อความพร้อมของผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองครั้งแรกก่อนจำหน่ายออกจากโรงพยาบาล


        รูปแบบการวิจัย: การศึกษาความสัมพันธ์เชิงทำนาย


        วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ดูแลของผู้ป่วยโรคหลอดเลือดสมองครั้งแรก จำนวน 108 คน ที่มารับการรักษาในโรงพยาบาลตติยภูมิแห่งหนึ่ง จังหวัดกรุงเทพมหานคร เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความพร้อม แบบสอบถามความวิตกกังวลและอาการซึมเศร้า และแบบประเมินระดับการพึ่งพา วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและการวิเคราะห์ถดถอยแบบเป็นขั้นตอน


        ผลการวิจัย: ผลการวิจัยพบกลุ่มตัวอย่างมีความพร้อมในการดูแลโดยรวมเฉลี่ยอยู่ในระดับปานกลาง (Mean = 21.5, SD = 6.2) ส่วนใหญ่มีความวิตกกังวลอยู่ในระดับน้อย ร้อยละ 64.8 มีอาการซึมเศร้าอยู่ในระดับน้อย ร้อยละ 66.7 เกือบครึ่งมีการรับรู้ความเพียงพอของรายได้ที่เพียงพอและเหลือเก็บ ร้อยละ 41.7 และหนึ่งในสามของผู้ป่วยมีระดับการพึ่งพามากที่สุด ร้อยละ 34.3 โดยพบว่าความวิตกกังวลของผู้ดูแลสามารถอธิบายความแปรผันของความพร้อมของผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองครั้งแรกได้ร้อยละ 19 และความวิตกกังวลของผู้ดูแลเพียงปัจจัยเดียวที่มีอิทธิพลต่อความพร้อมของผู้ดูแลผู้ป่วยโรคหลอดเลือดสมองครั้งแรกก่อนจำหน่ายออกจากโรงพยาบาล อย่างมีนัยสำคัญทางสถิติ (β = - .43, p < .05)


        สรุปและข้อเสนอแนะ: พยาบาลควรมีการประเมินระดับความวิตกกังวลของผู้ดูแลเพื่อวางแผนให้การช่วยเหลือ และควรพัฒนาโปรแกรมการพยาบาลเพื่อช่วยลดความวิตกกังวลของผู้ดูแล ซึ่งจะส่งผลให้ผู้ดูแลของผู้ป่วยโรคหลอดเลือดสมองครั้งแรก มีความพร้อมในการดูแลผู้ป่วยโรคหลอดเลือดสมองก่อนจำหน่ายออกจากโรงพยาบาลมากขึ้น
