Rate and Associated Factors for Falls among Elderly People: Chaopraya Waterfront Community in Nonthaburi Province

Abstract

Purpose: To determine the rate of falls and associated factors for falls among elderly people living in Chaopraya waterfront community.

Design: Descriptive research.

Methods: Eighty seven elders living in Chaopraya waterfront community in Nonthaburi province were selected using a convenience sampling method. Data were collected using a questionnaire asking demographic characteristics, history of illness, history of falls, and home environment and clinical examination determining blood pressure, visual acuity, gait and balance evaluation and body mass index.

Main findings: When asking about falling during the past 6 months, 16 % elders recalled at least one incident of falling and 8% elders were repeated falls. 33.3% elders reported falls in the past. 22.9% elders fell 1-2 times. The prevalence rate of falls was 41.3%. Number of falls in women was four times higher than men. There was relationship between falls and history of hyperlipidemia (χ2 = 5.06, p = .03). Elders with history of hyperlipidemia were less likely to fall than were elders with no history of hyperlipidemia. Elders with no history of hyperlipidemia had higher average age and greater number of females than were elders with history of hyperlipidemia.

Conclusion and recommendations: Fall prevention programs need to be implemented especially in elderly women with no history of hyperlipidemia. Health care providers should promote people in this group to detect medical conditions and other hidden fall risk factors to receive appropriate health intervention and management.


อัตรา และปัจจัยที่มีความสัมพันธ์กับการหกล้มของผู้สูงอายุ: ชุมชนริมแม่น้ำเจ้าพระยา จังหวัดนนทบุรี

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาอัตราการหกล้มและปัจจัยที่มีความสัมพันธ์กับการหกล้มของผู้สูงอายุชุมชนริมแม่น้ำ

รูปแบบการวิจัย: การศึกษาเชิงบรรยาย

วิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้สูงอายุที่อาศัยอยู่ริมแม่น้ำเจ้าพระยา จำนวน 87 คน จากการคัดเลือกตามความสะดวก โดยใช้แบบสอบถามข้อมูลทั่วไป ประวัติการเจ็บป่วย ประวัติการล้ม สิ่งแวดล้อมภายในบ้าน และการประเมินร่างกาย คือ ความดันโลหิต การมองเห็น การทรงตัว การเคลื่อนไหว และภาวะโภชนาการ

ผลการวิจัย : ผู้สูงอายุเคยหกล้ม 6 เดือนที่ผ่านมาร้อยละ 16 หกล้มซ้ำร้อยละ 8 เคยหกล้มในอดีตร้อยละ 33.3 หกล้ม 1-2 ครั้ง ร้อยละ 22.9 เคยหกล้มทั้งปัจจุบันและอดีต ร้อยละ 41.3 ผู้หญิงหกล้มมากกว่าผู้ชาย 4 เท่าพบความสัมพันธ์ของการหกล้มกับประวัติภาวะในเลือดสูง (χ2 = 5.06, p = .03) พบว่า ผู้ที่มีประวัติภาวะไขมันในเลือดสูงล้มน้อยกว่ากลุ่มไม่มีประวัติ ซึ่งในกลุ่มที่ไม่มีประวัติภาวะไขมันในเลือดสูงมีอายุเฉลี่ยสูงกว่าเล็กน้อย ส่วนใหญ่เป็นเพศหญิง

สรุปและข้อเสนอแนะ: ควรส่งเสริมป้องกันการหกล้มโดยเฉพาะในเพศหญิง ผู้ที่ให้ข้อมูลว่าไม่มีประวัติภาวะไขมันในเลือดสูงอาจไม่เคยคัดกรองสุขภาพ ขาดความระมัดระวังในการดูแลตนเอง หรือมีปัจจัยเสี่ยงอื่นๆ จึงควรมีการประเมินภาวะเสี่ยงเพิ่มเติมเพื่อให้ได้รับการดูแลที่เหมาะสม
