Effects of Motivational Counseling on Readiness for Long-Term Contraceptive Use in Pregnant Adolescents: A Pilot Study

Abstract

Abstract

Purpose: To study effect of motivational counseling on readiness for long-term contraceptive use in pregnant adolescents and to determine the feasibility and appropriateness of motivational counseling on long-term contraceptive use.

Design: One-group pre-test post-test design.

Methods: The guidelines of motivational counseling for long term contraceptive use in accordance to the stage of readiness was developed and pilot tested in 30 pregnant adolescents who sought antenatal care at the adolescent antenatal clinic in a University Hospital in Bangkok, between October 2014 and April 2015. The readiness for long-term contraceptive use was evaluated before and after motivational counseling. Wilcoxon Signed-Rank Test was employed in data analysis, and the steps in provision of motivational counseling were also analyzed by content analysis.

Main findings: The findings revealed that the median score of readiness for long-term contraceptive use after motivational counseling was higher than that before motivational counseling with statistical significance (Z = - 3.566, p < .05). The result from content analysis found that having a clear life goal and influence from significant family member also affect their decision on long-term contraceptive use. The motivation to use long-term contraception for most participants are going back to study and when weighing the benefit of implant contraception is more than pain from multiple injection and suffering from having another baby.

Conclusion and recommendations: Motivational counseling could help moving more pregnant adolescents into the stage of preparation toward long-term contraceptive use. Motivational counseling should be provided during 32 to 36 weeks of gestational ages, 1 to 2 sessions, and approximately lasting 30 minutes in the first session and 10 to 15 minutes in the second session. Family members should be allowed to attend counseling sessions.

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาผลของการให้การปรึกษาเพื่อสร้างแรงจูงใจต่อความพร้อมในการใช้การคุมกำเนิด ระยะยาวในสตรีตั้งครรภ์วัยรุ่น และความเป็นไปได้ของการให้การปรึกษาเพื่อสร้างแรงจูงใจในการใช้การคุมกำเนิดระยะยาว

รูปแบบการวิจัย: การวิจัยแบบกลุ่มเดียววัดผลก่อนหลัง

วิธีการดำเนินการวิจัย: พัฒนาแนวปฏิบัติการให้การปรึกษาเพื่อสร้างแรงจูงใจต่อการใช้การคมกำเนิดระยะยาวตามระดับของความพร้อม นำไปทดลองใช้ในกลุ่มตัวอย่างเป็นสตรีตั้งครรภ์วัยรุ่นที่มาฝากครรภ์ที่คลินิกฝากครรภ์สำหรับสตรีตั้งครรภ์วัยรุ่น โรงพยาบาลศิริราช ระหว่างเดือน ตุลาคม 2557 - เมษายน 2558 จำนวน 30 ราย ประเมินระดับความพร้อมในการคุมกำเนิดระยะยาวก่อนและหลังการให้การปรึกษา วิเคราะห์เปรียบเทียบระดับความพร้อมก่อนและหลังให้การปรึกษาด้วยสถิติ Wilcoxon Signed - Rank Test และวิเคราะห์ขั้นตอนในการให้การปรึกษาด้วยการวิเคราะห์เนื้อหา

ผลการวิจัย: ค่ามัธยฐานคะแนนความพร้อมในการคุมกำเนิดระยะยาวภายหลังให้การปรึกษามากกว่าก่อนให้การปรึกษาอย่างมีนัยสำคัญทางสถิติ Z = -3.566, p = .001 และจากการวิเคราะห์เนื้อหาพบว่า การมีเป้าหมายชีวิตที่ชัดเจนและอิทธิพลจากบุคคลสำคัญในครอบครัวมีผลต่อการตัดสินใจของสตรีวัยรุ่นในการคุมกำเนิดระยะยาว และสิ่งที่เป็นแรงจูงใจในการคุมกำเนิดระยะยาว คือ ต้องการกลับไปศึกษาต่อ และการประเมินว่าการฝังยาดีกว่าต้องเจ็บจากการฉีดยาหลายครั้ง หรือต้องลำบากจากการมีบุตรอีกคน

สรุป และข้อเสนอแนะ: การปรึกษาเพื่อสร้างแรงจูงใจช่วยให้สตรีตั้งครรภ์วัยรุ่นเข้าสู่ระยะพร้อมที่จะปฏิบัติในการคุมกำเนิดระยะยาวมากขึ้น โดยเริ่มให้การปรึกษาเมื่ออายุครรภ์ 32-36 สัปดาห์ จำนวน 1-2 ครั้ง โดยใช้เวลาประมาณ 30 นาที ในครั้งแรก และ 10-15 นาทีในครั้งที่สอง ทั้งนี้ผู้ให้การปรึกษาควรช่วยให้สตรีตั้งครรภ์วัยรุ่น ค้นหาเป้าหมายในชีวิตที่ชัดเจน เช่น การกลับไปเรียน และตระหนักถึงความเสี่ยงที่จะตั้งครรภ์ซ้ำเพื่อเป็นแรงจูงใจในการใช้การคุมกำเนิดระยะยาว
