Abstract

This study aims to determine the distance and direction of the external reference pin (ERP) at nasion. The cone-beam computed tomography (CBCT) data of 126 patients undergoing orthognathic surgery in 2017-2018 were collected. For the thickness of the structure, A-plane and B-plane were created. A-plane was drawn from sella to hard tissue nasion and B-plane was created from soft tissue nasion to the shortest distance to anterior cranial fossa (ACF). The direction of ERP was determined between B-plane and nasal dorsum. For the results, Plane A distance of soft tissue to ACF was 20.63 ± 4.97 mm, Plane B distance of soft tissue nasion to the shortest distant to ACF was 16.31 ± 2.64 mm. The patients with absence of frontal sinus had less distance of soft tissue nasion to the shortest distance to ACF (B_ShortACF = 13.46 ± 5.33 mm). The overall insertion angle was 29.64 ± 10.53 degrees. Finally, the ERP can be used more safely following the recommendations of previous studies as well as this study. First, always find the frontal sinus
because the absence group has shorter distance. Second, the direction of ERP should be more than 30 degrees by using nasal dorsum as a reference plane to increase the distance.
