Practial Use of Non-Vitamin K Antagonist in Atrial Fibrillation
Abstract
ยาต้านการแข็งตัวของเลือดกลุ่มใหม่หรือ Non-vitamin K Antagonist (NOAC) ประกอบด้วยยาในกลุ่ม Direct Thrombin Inhibitor (Dabigatran) และ Factor Xa inhibitor (Rivaroxaban, Apixaban, Edoxaban) มีหลักฐานเชิงประจักษ์ว่า NOAC สามารถป้องกันการเกิด Stroke ในผู้ป่วย Atrial Fibrillation ได้ไม่ต่างกับ Warfarin หรืออาจจะเหนือกว่า ข้อดีที่เด่นชัดกว่า Warfarin คือ ลดความเสี่ยงในการเกิดเลือดออกในสมองและไม่ต้องเจาะเลือดดูระดับ INR1อย่างไรก็ตาม NOAC เป็นยาที่ออกฤทธิ์เร็วหมดฤทธิ์เร็วจำเป็นต้องกินยาอย่างเคร่งครัด หากลืมกินยาจะมีความเสี่ยงในการเกิด Stroke มากขึ้น American College of Cardiology (ACC) American Heart Association (AHA) Heart Rhythm Society (HRS)2 และ European Society of Cardiology (ESC)3แนะนำให้ใช้ NOAC เป็นตัวเลือกแรกหรือทดแทน Warfarin โดยเฉพาะในผู้ป่วยที่ไม่สามารถควบคุมระดับการแข็งตัวของเลือดได้เข้าเป้า NOAC เป็นยากลุ่มใหม่ ประสบการณ์ใช้ยาในประเทศไทยยังไม่มากเท่า Warfarin ขนาดยา กลไกการออกฤทธิ์ คุณสมบัติทางเภสัชศาสตร์ ปฏิสัมพันธ์กับยากลุ่มอื่น การจัดการกับภาวะเลือดออก รวมถึงการใช้ยาในกรณีเฉพาะต่างๆ เช่น การใช้ NOAC ร่วมกับยาต้านเกล็ดเลือดในผู้ป่วยหลังทำการซ่อมแซมหลอดเลือดหัวใจ การใช้ NOAC ในกรณีที่จำเป็นต้องทำการกลับจังหวะหัวใจ การใช้ NOAC ในกรณีที่ทำ AF ablation หรือ Left Atrial Catheter Ablation มีรายละเอียดที่แตกต่างจาก Warfarin วิพัชร พันธวิมล พ.บ.หน่วยโรคหัวใจ กลุ่มงานอายุรกรรม โรงพยาบาลตำรวจพีรพัฒน์ เกตุค้างพลู พ.บ.หน่วยสรีรวิทยาไฟฟ้าหัวใจ กลุ่มงานอายุรศาสตร์หัวใจ สถาบันโรคทรวงอกPractical Use of Non-Vitamin K Antagonist in Atrial Fibrillationวิธีใช้ยาต้านการแข็งตัวของเลือดกลุ่มใหม่ในผู้ป่วยโรคหัวใจห้องบนสั่นพลิ้ววิธีใช้ยาต้านการแข็งตัวของเลือดกลุ่มใหม่ในผู้ป่วยโรคหัวใจห้องบนสั่นพลิ้วPractical Use of Non-Vitamin K Antagonist in Atrial Fibrillationแพทย์และพยาบาลรวมถึงบุคลากรทางการแพทย์จำเป็นต้องเรียนรู้ เพราะปัจจุบันมีการใช้ยา NOAC มากขึ้นเรื่อยๆ ในประเทศไทย
