อัตรารอดชีพและปัจจัยพยากรณ์การรอดชีพผู้ป่วยโรคมะเร็งช่องปากชนิด Squamous Cell Carcinoma

ภูมิหลัง: มะเร็งช่องปากชนิด squamous cell carcinoma (SCC) เป็นมะเร็งที่เกิดจากเซลล์เยื่อบุผิวในช่องปาก พบบ่อยในผู้ใหญ่และมีอัตราการเกิดสูงในหลายประเทศ รวมถึงประเทศไทย วัตถุประสงค์: เพื่อศึกษาอัตราการรอดชีพ และปัจจัยพยากรณ์การรอดชีพของผู้ป่วยมะเร็งช่องปากชนิด SCC ที่เข้ารับการรักษาในโรงพยาบาลร้อยเอ็ด วิธีการ: การศึกษาแบบ retrospective cohort study โดยเก็บข้อมูลจากเวชระเบียนผู้ป่วยระหว่าง 1 มกราคม 2558 ถึง 31 ธันวาคม 2564 ติดตามจนถึง 31 ธันวาคม 2566 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Kaplan-Meier survival method และ Cox proportional hazard model (p < .05) ผล: ผู้ป่วย 273 ราย ส่วนใหญ่เป็นเพศหญิง (69.37%) อายุเฉลี่ย 66.14 ปี (±12.78) ติดตามรวม 1,116 คน-เดือน พบผู้เสียชีวิต 222 ราย อัตราอุบัติการณ์ 19.87 ต่อ 100 คน-เดือน (95%CI: 12.23, 23.75) มัธยฐานการรอดชีพ 1.66 ปี อัตรารอดชีพ 1, 3, 5 และ 10 ปี เท่ากับ 62.89%, 42.66%, 32.73% และ 19.93% ตามลำดับ ปัจจัยเสี่ยงต่อการเสียชีวิต ได้แก่ histology grading เป็น poorly differentiated (adjHR = 2.91; 95%CI: 1.61, 7.22) และมะเร็งระยะที่ 2 (adjHR = 1.90; 95%CI: 1.19, 3.00), ระยะที่ 3 (adjHR=2.65; 95%CI: 1.62, 4.32), และระยะที่ 4 (adjHR = 3.71; 95%CI: 2.13, 6.47) สรุป: ผู้ป่วยมะเร็งช่องปากที่โรงพยาบาลร้อยเอ็ดมีมัธยฐานการรอดชีพ 1.66 ปี และอัตรารอดชีพ 5 ปี 32.73% โดยลักษณะทางเซลล์วิทยาและระยะโรคที่รุนแรงเป็นปัจจัยพยากรณ์ที่ลดอัตราการรอดชีพของผู้ป่วย
