Development a Care System for Stroke Patients from Hospital to the Patients’ Home
Abstract
The objective of this action research is to develop a home care system for stroke patients from Rayong Hospital to the patients’ home and effectiveness evaluation. A methodology of PAOR was conducted during 2019-2022. The topics for focus groups and structure interview was used as a tool for collecting data and the content analysis were used for brainstorming in a group discussion. The content analysis and descriptive statistics, Paired-Samples T test and One-Way ANOVA were used to analyze the data. The 20 stakeholders and 174 stroke patients with care givers were invited to participate in the study. It was found the mean scores of the behaviors, support from peer, and satisfaction toward the new care system was statistical significance higher (p value <0.05). There was no change in attitude. In concluding, the new care system working by a multidisciplinary team helped increasing in behavior of self-care, supporting form peer, and enhance the patient satisfaction However, continuing an effective and efficient collaborative practice for the next individual stroke patient require improvement of health outcomes at their home.
