Effects of Flipped Classroom Combined with Case-Based Learning on Knowledge and Self-Directed Learning Characteristics among Third-Year Nursing Students in the Topic of Nursing Care for Older Adults with Geriatric Syndromes

Introduction: Nursing care for older adults with geriatric syndromes is a content-dense topic in which conditions present with overlapping manifestations. Lecture-based instruction alone may be insufficient to develop applied knowledge and self-directed learning (SDL) characteristics among nursing students.
Aim: To compare knowledge and SDL characteristics among nursing students before and after a flipped classroom combined with case-based learning (FC-CBL) approach.
Methods: A quasi-experimental, one-group pretest-posttest design was employed. The entire population of 74 third-year nursing students enrolled in the Gerontological Nursing course in the 2025 academic year participated. The intervention comprised four instructional hours over two weeks. Data were collected between February and March 2026 using a 20-item multiple-choice knowledge test and a 50-item, 8-dimension SDL questionnaire; item-level content validity indices ranged from 0.67 to 1.00 (S-CVI/Ave = 0.91), and reliability was KR-20 = 0.86 and Cronbach’s alpha = 0.90, respectively. Data were analyzed using descriptive statistics and paired t-tests at the .05 level.
Results: Post-intervention knowledge scores (M = 16.45, SD = 2.73) were significantly higher than pre-intervention scores (M = 13.12, SD = 2.24) (mean difference 3.33; t(73) = 7.186, p < .001, Cohen’s d = 0.84). Overall SDL scores also increased, from 175.14 (SD = 24.28) to 208.68 (SD = 28.46) (mean difference 33.54; t(73) = 12.948, p < .001, d = 1.51). All eight SDL dimensions improved significantly (p < .001).
Conclusion and recommendations: Nursing students demonstrated higher knowledge and SDL scores than before the intervention. Because the study lacked a control group, causal inference cannot be drawn. FC-CBL may be applied to content-dense nursing topics using pre-class materials, quality-appraised case studies, and reflective in-class activities.
